    Taman Kili Kili Beach is a nesting place for turtles. There are Olive Ridley (Lepidochelys olivacea), Hawksbill Turtle (Deretmocelys imbricate), Green Turtle (Chelonian mydas)and Leatherback (Dermocelys oliacea) that found completing their natal homing to this area during the night. The beach area was conserved and managed by a group of local community that called as Pokmaswas Taman Kili-Kili Beach with consideration of the government at the village level.     Bronchiectasis is a chronic airway disease with abnormal and persistent bronchial dilatation caused by a variety of reasons. In recent years, numerous reports have shown that bronchiectasis is heterogeneous, the clinical characteristics of patients with different phenotypes are different, and the efficacy of a treatment regimen may vary greatly in patients with different bronchiectasis phenotypes. This paper summarizes the current clinical phenotypic classification of bronchiectasis from the perspective of etiology, microbiology, and the frequency of acute exacerbation, and cluster analysis was used to determine new clinical phenotypes and their statistical and clinical significance. Different tools for assessing disease severity yield different outcomes. This article summarizes the research progress in the above areas, hoping to provide a more comprehensive understanding of the disease.

    Epidural fibrosis is a challenging topic in spinal surgery. Numerous clinical and experimental studies have been focused on this issue to clarify problems faced in spinal procedures for the patient as well as the surgeon and find out new methodologies. Dense cytokines and growth factors which are released from inflammatory cells have been suggested to play a major role in the inception and progression of fibrosis. One of the most investigated and important actor in epidural fibrosis is assumed to be the transforming growth factor-1β (TGF-1β) formation. Studies showed that Dexmedetomidine (DEX) downregulates TGF-β pathway with its anti-inflammatory and antioxidant effects. From this point of view, for the first time in the literature we try to observe if there will be an effect of topical DEX administration over epidural fibrosis in a rat model. We hypothesized that DEX might have preventive effects on epidural fibrosis via anti-inflammatory and antioxidant effects. Twenty-four adult male Wistar albino rats were randomly assigned to three groups (Topical DEX, Spongostan, Laminectomy). A total laminectomy was performed at the L3-L5 level and then the ligamentum flavum and epidural fat tissue were cleared away from the surgical site. Histopathological assessment was performed postoperatively after 4 weeks. Our study revealed that topical DEX administration may have effects on reducing epidural fibrosis. Topical DEX administration may be helpful in preventing epidural fibrosis after laminectomy in rats through multiple anti-inflammatory and antioxidant mechanisms as well as through TGF -1β pathway.

    At Indonesia, cases of residential house fires are still rampant. This resulted in considerable losses for the population of Indonesia. If there is no prevention or countermeasure, it is possible that the danger of a house fires will continue. Therefore, this system exists to create a condition where the system is able to detect the potential that will bring a fire hazard. In this system using a method that is the application of a multisensory system in detecting the presence of fire, smoke and temperature in the room. The sensors used include KY-026 fire sensor, MQ-9 smoke sensor and DS18B20 temperature sensor. Then the system also implements an intelligent system that is fuzzy logic to process sensor reading data. The three sensor inputs will be processed through the fuzzification stage, rule evaluation and the deffuzification.     Accurate, unbiased phenotypic performance of progeny is critical for the development of fish‐breeding programs, and identification of possible environmental factors affecting phenotypes is an important aspect of genetic analysis. Catfish progeny can be produced by either natural pond‐spawning or hormone‐induced strip‐spawning. We determined the effects of pond‐spawning versus strip‐spawning on fingerling weight, harvest weight, and carcass yield of channel catfish, Ictalurus punctatus, progeny. Progeny from 65 pond‐spawned full‐sib families and 29 strip‐spawned full‐sib families were reared in separate family tanks, then tagged as fingerlings, and then stocked communally in earthen ponds and harvested at ~17 months posthatch. A sample of fish from each full‐sib family was measured for carcass yield. Fingerling weight was not affected by sex or spawning type. Harvest weight was higher for males than for females and higher for pond‐spawned fish than for strip‐spawned fish. Carcass yield was higher for females than for males and higher for pond‐spawned fish than for strip‐spawned fish. Results indicate that strip‐spawning had substantial negative effects on progeny harvest weight and carcass yield. Accounting for the effect of strip‐spawning in breeding programs and the potential effects of strip‐spawning on commercial catfish production should be determined.

    Visit-to-visit blood pressure variability (VVBPV) is an independent risk factor for cardiovascular morbidity and mortality in the general population. Hemodialysis (HD) patients have a poor prognosis due to an increased prevalence of cardiovascular disease. Intradialytic hypotension is associated with excess mortality, but whether VVBPV influences mortality is still unclear in HD patients. The present study aimed to investigate the characteristics of VVBPV in these patients. A total of 324 maintenance HD patients, who could be followed for 60 months, were recruited. We used variation independent of the mean (VIM) in pre-dialysis systolic blood pressure (pre-VIM-SBP) as an index of VVBPV. We investigated (1) the reproducibility of pre-VIM-SBP, (2) the relationship between pre-VIM-SBP and background factors, and (3) the association between pre-VIM-SBP and mortality. Pre-VIM-SBP showed significant reproducibility [intraclass correlation, 0.45 (P < 0.001)]. Higher pre-VIM-SBP was associated with less physical activity and worse left ventricular diastolic function. Higher pre-VIM-SBP was associated with a higher rate of cardiovascular deaths independent of other factors. These data suggest that VVBPV in HD patients is reproducible and associated with various background factors. VVBPV is independently correlated with cardiovascular mortality (hazard ratio: 1.166, 95% confidence interval: 1.030-1.320, P = 0.015). Further studies are necessary to confirm the mechanism of increased VVBPV and to clarify whether reducing VVBPV will improve the prognosis for HD patients.

    Objective of this study is to assess the pullout performance of various pedicle screws in different test materials after toggling tests comparatively. Solid core, cannulated (cemented), novel expandable and solid-core (cemented) pedicle screws were instrumented to the polyurethane foams (Grade 10 and Grade 40) produced in laboratory and bovine vertebra. ASTM F543 standard was used for preparation process of samples. Toggling tests were carried out. After toggling test procedures, pullout tests were performed. Load versus displacement graph was recorded, and the ultimate pullout force was defined as the maximum load (pullout strength) sustained before failure of screw. Anteriosuperior and oblique radiographs were taken from each sample after instrumentation in order to examine screw placement and cement distribution. Listening to music was found to be the safest and most common non-drug treatment method. The aim of this study is to evaluate the effect of listening to music on adult patients in cast room procedures. This study points out the relation between anxiety and anxiety relevant cardiac arrhythmia. The study was performed on 199 patients with stable general condition, aged above 18. Polyurethane foams (Grade 10 and Grade 40) produced in laboratory and bovine vertebrae were instrumented with normal, cannulated (cemented), novel expandable and normal (cemented) pedicle screws.     One of the most common problems with transpedicular screws is screw pullout. This study was conducted to measure the pullout strengths of newly designed transpedicular screws. The design of the three group screws were conical cored standard pedicle screw (Type A), dual threaded pedicle screw (Type B), dual core and dual threaded pedicle screw (Type C), respectively. Polyurethane (PU) blocks in 25 mm and 50 mm thickness were used to investigate the effect of just the pedicle on pullout strength and both distal (vertebral body) and proximal (pedicle) parts of the screw. The screws were also tested in ovine lumbar vertebrae. Type C screw exhibited 5.9% and 12.9% higher pullout strength than Type A and Type B, and 15.4% and 8.6% higher pullout strength than Type A and Type B, respectively on 25 mm and 50 mm thick PU foam block. Here, we describe a new technic ‘spinolaminoplasty (Turkish Open-door laminoplasty)’ with newly designed titanium mini plate that placed one side to lateral mass, and the other on spinous process to keep the laminae opened. Two different fixation materials were used for axial compression tests. One was Ultra high molecular weight polyethylene block with cervical vertebrae geometry and fresh ovine cervical vertebrae. In the first group, mini plates were fixed on laminae as in the conventional method described by Hirabayashi.     Icteric patients with clinical and biochemical evidence of liver disease, admitted into various hospitals in Malaysia, were investigated to determine the cause of their infection. Of these patients, 11.0% (16/145) were found positive for IgM anti-HAV (EIA), 4.1% (6/145) for IgM anti-HBc (EIA), 1.0% (1/102) for IgM anti-CMV (ELISA), 17.2% (16/64) for rising titres of leptospiral agglutinin, and none for heterophile antibody of EBV. Hepatitis NANB accounted for 67.9% of cases. The mean serum transaminases (ALT and AST) values in patients with hepatitis A and B were higher (more than 500IU) than in patients with leptospirosis or non-A, non-B hepatitis, whereas serum bilirubin levels were higher in patients with hepatitis A and leptospirosis than in patients with hepatitis B.

    Sera were obtained from 494 non-icteric patients admitted with illnesses other than overt hepatitis into the medical wards of the rural and urban hospitals in Malaysia. They were tested for HBsAg, HBeAg, and anti-HBs by enzyme immunoassay. The overall HBsAg carrier rate was 18.0% ranging from 9.6% in children, (10 years and under), to a maximum of 23.5% in the adolescents (11 to 20 years), the rates decreasing subsequently to 16.5% and 20.8% in the adult and middle-age groups, respectively. The Chinese (18.6%) and Malays (19.9%) had similar HBsAg carrier rates but the rate in the Indians (9.0%) was distinctly lower. Similar rates were observed in the males (16.5%) and the females (19.8%). The carrier rate was 17.1% in rural patients compared with 21.4% in the urban ones. The 'e' antigen was found in 14 of the 89 HBsAg carriers (15.7%).
