During the last three decades, many Dutch soldiers have actively participated in UN missions in war zones, for example in Lebanon, Afghanistan and Mali. After their active service a number of them suffered from coping and adjustment problems such as loneliness, depression, and PTSD. (Motshagen 2019, Reijnen & Duel, 2019). To find support for these problems, they can appeal to the National Veterans Care Organisation (LZV) for medical, psychosocial and psychotherapeutic support. (NVI, 2021, 2020). Psychosocial care is organised regionally in a stepped care regional network, involving military and civilian assistance organisations. The services provided range from general social work to highly specialised trauma treatments. There are regular network meetings at every level, management, directors, and support professionals. For the latter group, a chain intervision model was developed based on my group supervision model. (Haans & Balke 2017). Each participating organisation sends two employees who, in rotation for at least two years, participate in a group intervision session, held three times a year. Each peer review group is supervised by two permanent intervisors. This format has been elaborated over the past eight years. The original model was modified from case supervision to chain intervision, focussing on cooperation between staff from different institutions in a particular region. During the Corona pandemic, the intervision sessions were held online. Short scheme of Identification Group Intervision 1. Clinical supervision has been known for decades. However, only in the past decade have training courses for supervision been developed and offered for health professionals working with a severely traumatized client population. Health professionals working as supervisors in this field are faced with specific problems. Together with Johan Lansen and Ton Haans, the Berlin Treatment Center for Torture Victims (bzfo) adapted the training method developed by Lansen/Haans in 1999, which offers participants a structural framework. In cooperation with the German Society for Supervision (DGSv), the bzfo offers this training course in Germany where it takes one and a half years to complete. To meet the vast need for supervision in countries where health professionals work under difficult and adverse conditions, the bzfo is now in contact with colleagues in the Gaza region with a proposal for supervision tailored to their specific needs.

Samenvatting  Sinds vijftien jaar heb ik contact met transculturele hulpverleners uit verschillende landen. Wanneer ik met Canadezen in aanraking kom, bekruipt me een gevoel van verwarring: het is allemaal zo genuanceerd wat ze denken. Wanneer je als Europeaan denkt te snappen wat ze bedoelen, dan is er weer een uitzondering op de regel. Ze lijken politiek hypercorrect te zijn. Dit ervoer ik ook weer bij het eerste uur van de workshop die de Canadese cultureel psychiater Laurence Kirmayer gaf in ’s-Hertogenbosch over het onderwerp ‘Contributions of cultural psychiatry to understanding and working with refugees, victims of trauma and forced migration’.

ABSTRACT Trauma-informed group supervision is a novel way of providing much-needed supervision to trauma helpers. We describe a structured group supervision method originally developed for Western trauma supervisors. Core features of the method are two identification rounds: first, identification with the client as a person and, second, identification with the position of the case presenter. We explain why we designed this structured identification approach and elaborate its main features. This method has been applied in many intercultural contexts and post-conflict regions of the world. Finally, we formulate some basics of intercultural application and contracting for group supervision.

Supervision ist eines der anerkannten Verfahren zur Unterstützung von professionellen Helfern im psychiatrischen und psychosozialen Bereich. Gegenstand dieses Artikels wird der Bereich der Supervision sein, bei der Supervisor und Supervisand in einer gemeinsamen Reflektion die Arbeit des letzteren betrachten. Ziel ist es, mehr kognitive, emotionale und methodische Kompetenz und Handlungsfähigkeit zu erwerben. In der Rekonstruktion des Behandlungsverlaufes reflektieren Supervisor und Supervisand die Bedeutung des bisherigen Prozesses, den eigenen Anteilen daran, welche Interpretationen erfolgten und welche Gefühle den Prozess begleiteten - ein Prozess, der in hohem Masse von Intuition und gemeinsamen Grundannahmen geleitet wird, in anderen Worten, von der gemeinsamen Kultur. Treffen im Supervisionsprozess unterschiedliche Kulturen aufeinander, so ergeben sich besondere Fragen: Welchen Einfluss haben diese kulturellen Unterschiede auf den interpretativen Prozess? Und müssen sie nicht zwangsläufig zu Missverständnissen führen?

This summary is divided in three parts: personal workload (chapter 1 - 3), team workload (chapter 4), and regulation of personal and team workload (chapters 5 and 6). Personal client-related workload encompasses countertransference resulting from direct client contacts and characteristics of cross-cultural, trauma-related countertransference, such as the gender, culture and trauma history of the caregiver. Chapter 3 contains a critical review of presumed long term effects, such as secondary traumatisation and vicarious traumatisation. It also describes relevant European research in this domain.
The chapter about team workload phenomena, includes parallel processes and team conflicts, symbiotic and fragmented teams. These are related to individual cognitive schemas and work satisfaction.
In the chapters about regulation of client-related workload, individual and team strategies such as intervision, supervision and regulation of parallel processes are described.

If helpers want to understand their clients they apply different activities. Often a Cultural Understanding by Documentation of Living conditions and Understanding of Suffering. But survivors of violence are more victims for other cultures. In their new countries they face other existential tasks which they encounter in different coping styles. To understand their complicated living conditions helpers must implement these coping stiles and combine these with the resources and actual context in their support. Systemic support is than the most appropriate mode of activity.

The project aimed to train staff of psychosocial support organizations for Iraqi refugees in Amman in community based interventions, improving communication skills and stimulating mutual cooperation.    Families and households are the primary focus.They are approached in their specific context with its challenges and opportunities.  They are approached in their specific context with its challenges and opportunities.  Tools are applied to map the families and to stimulate them to take ownership.Involved local aid organizations are explored and delineated with easy tools to avoid overlap.  Ethical implications are addressed in implementing the interventions.
