Objetivo: Realizou-se estudo clínico prospectivo para verificar se há associação da assimetria do acometimento corneano no ceratocone com a posição preferencial de dormir. RESULTADOS: Todos os participantes foram submetidos ao questionário para avaliação da posição preferencial de dormir. Classificou-se como "olho dependente" o olho posicionado inferiormente durante o sono, independentemente do decúbito (lateral ou ventral), e de "olho não dependente" o olho posicionado superiormente, tanto em decúbito lateral quanto ventral. O presente estudo mostrou maiores alterações topográficas nos "olhos dependentes" com ceratocone que podem estar relacionadas com a posição de dormir. Nas posições de decúbito lateral direita ou esquerda observou-se maior deformação da córnea no "olho dependente" com ceratocone e este achado poderia ser uma resposta à traumas mecânicos originados durante o sono e secundários à posição preferencial de dormir. O uso intracameral de antibiótico no final da cirurgia da catarata, em comparação com o uso tópico de antibiótico no pré e/ou pós-operatório, reduz significativamente o risco de endoftalmite pós-cirúrgica. O uso profilático desse regime terapêutico já é prática corrente na Europa e em número muito expressivo de países de outras regiões do mundo. Não é adotado de forma rotineira nos Estados Unidos porque lá não há preparação comercial de antibiótico para uso intracameral aprovada pelo United States Food and Drug Administration (FDA). A Sociedade Brasileira de Cirurgia Refrativa e Catarata (ABCCR/BRASCS), com esta atualização sobre o tema pretende mostrar o impacto positivo do uso intracameral de antibiótico na redução da taxa de endoftalmite pós-cirúrgica de catarata e, ao mesmo tempo, ressaltar as evidências de boa qualidade que reforçam a segurança e a eficácia do uso desse regime terapêutico profilático.

No Brasil, a maior barreira para o uso intracameral de antibiótico profilático rotineiro no final da cirurgia de catarata é a falta de uma preparação comercial de antibiótico para uso intracameral aprovada pela Agência Nacional de Vigilância Sanitária (ANVISA). O Grupo de Revisão da Literatura da Sociedade Brasileira de Catarata e Cirurgia Refrativa (ABCCR/ BRASCS) escolheu este tema para atualização pretendendo revisar aspectos dos espectros de atividade, dosagem e preparação, segurança e eficácia dos antibióticos moxifloxacino, cefuroxima e vancomicina, considerados os mais seguros para o uso intracameral.

Dry eye disease (DED) is common in Rheumatoid Arthritis (RA) patients. The application of conjunctival goblet cell count as a clinical biomarker to diagnose and respond to treatment can take place in rheumatoid arthritis patients under TNF-inhibitors (TNFi) therapy. This study aimed to investigate the ocular surface parameters and the long-term effects of TNFi therapy on ocular surface features and goblet cell count of rheumatoid arthritis patients. At baseline, rheumatoid arthritis patients eligible to TNFi were compared to healthy controls (similar age/gender), regarding Ocular Surface Disease Index (OSDI) questionnaire, Schirmer I test, tear break-up time test, vital dye staining of the ocular surface, and conjunctival impression cytology. Dry eye disease can compromise the patient’s quality of life. Few studies assessed the ocular surface (OS) in Ankylosing Spondylitis (AS) patients. This study aimed to evaluate the clinical and cytological findings of the OS in patients with AS, classify dry eye disease (DED) severity grade and conjunctival impression cytology (IC), and the effects of TNF inhibitors (TNFi) in a one-year follow-up. A baseline (BL) evaluation included 36 AS patients and 39 healthy controls. They fulfilled the Ocular Surface Index Disease questionnaire and underwent the Schirmer I test, break-up time, vital staining, and conjunctival IC. A DED severity grade, as well as IC rating, was applied. PURPOSE To assess the full thickness macular hole (FTMH) size using the choroidal hypertransmission signal on spectral domain optical coherence tomography (SD-OCT), and to compare this method to the standard aperture measurement of the minimum aperture size at the level of the neurosensory retina. DESIGN Cross-sectional study of retrospective data. METHODS Eyes with FTMH imaged on SD-OCT were included. Two independent masked graders used the device's built-in caliper tool to measure the FTMH minimum aperture size at the level of the neurosensory retina, and the size of the corresponding hypertransmission signal below the level of the retinal pigment epithelium (RPE)/ Bruch's membrane complex. To assess the reproducibility of the hypertransmission measurement in tilted scans, two measurements were obtained and compared; the first was traced parallel to the RPE (parallel-hypertransmission), and the second was horizontal to the image frame (horizontal-hypertransmission), both using Image J software. RESULTS A total of 31 eyes were enrolled. The mean FTMH minimum aperture size was smaller compared to both the choroidal parallel-hypertransmission and horizontal-hypertransmission measurements (mean ± SD: 335.7 ± 139.5 μm, 376.7 ± 150.6 μm, 375.1 ± 150.0 μm, respectively. P<0.001 for both comparisons). CONCLUSION The proposed hypertransmission measurement is a feasible and reproducible alternative to assess FTMH size and could provide the basis for an automated FTMH measurement on cross-sectional SD-OCT scans, as presented in this study, or on the SD-OCT volumetric dataset by using an en face projection.

Purpose To report a case of post-LASIK corneal ectasia due to a thick flap, while the contralateral eye did not develop ectasia after an incomplete deep flap cut, followed by a thinner flap LASIK procedure. Methods Case report Results This 45 years old female patient had bilateral myopic LASIK in 1999. Preoperative anterior curvature map was regular with no signs of keratoconus. Central keratometry was 42.88 × 44.70 @ 163 in OD and 43.43 × 45.24 @ 175 in OS. Ultrasound central corneal thickness was 586 μm and 619 μm in the right eye and left eye, respectively. Corneal OCT identified a deep meniscus-shaped LASIK flap, with a central thickness of a 392 μm in the right eye, and an incomplete deep peripheral cut in the left eye with a thinner meniscus-shaped LASIK flap. Conclusion Unilateral ectasia after LASIK may occur due to a thick flap which leads to biomechanical failure of the cornea.
