LASIK

This study assesses the agreement between intraocular pressure (IOP) measurements taken with the Goldmann applanation tonometer (GAT) and a new experimental applanation tonometer with a convexly shaped apex (CT) after laser myopic refractive surgery.
Two different CT radii (CT1 and CT2) were designed with a finite element analyser, and a prospective double masked study on 102 eyes from 102 patients was carried out. A Bland-Altman plot and intra-class correlation coefficient (ICC) were calculated to assess the agreement between GAT measurements and the measurements of both CT1 and CT2 before and after myopic laser assisted in situ keratomileusis (LASIK; n=73) and photorefractive keratectomy (PRK; n=29). We evaluated a subset of two subgroups (n=36 each) for intra and inter-observer (IA/IE) error.
From the whole cohort, the best IOP agreement was observed between GATpre and CT1post surgery: 16.09 ± 2.92 vs 16.42 ± 2.87 (p<0.001); ICC = 0.675 (95% CI: 0.554 - 0.768). In the analysis of LASIK vs PRK, GATpre and CT1post showed the highest agreement, although LASIK measurements were more accurate than PRK, as the ICC = 0.718 (95% CI: 0.594 - 0.812) and ICC = 0.578 (95% CI: 0.182 - 0.795) respectively.
Excellent agreement was observed for IA/IE, and there was an ICC > 0.8 (95% CI) in all cases. CT1 proved more accurate in the LASIK subgroup.
In conclusion, our new version of GAT could be used with post-surgery LASIK patients as a more accurate measurement device compared to the current reference tonometer.

INTRODUCCIÓN/MATERIAL Y MÉTODOS. Presentamos el caso de un paciente de 63 años operado de catarata en ambos ojos 10 años después de un LASIK de alrededor de 6 dioptrías de miopía que desarrolló un sÍndrome de fluido en la interfase bilateral inducido por corticoides.

RESULTADO/ DISCUSIÓN. El síndrome de fluido en la interfase inducido por corticoides es un cuadro ampliamente descrito en la literatura. Se trata de un acúmulo de líquido en la interfase creada tras un LASIK, que se produce por un aumento de la tensión ocular. Se puede confundir fácilmente con una DLK debido a las falsas lecturas de la tensión ocular dadas por la tonometría y a que le resistencia de la cornea superficial se ve disminuida por el líquido subyacente. En el caso presentado ocurre incluso 10 años después de la intervención de LASIK.

PURPOSE: To investigate the incidence, culture results, risk factors, and visual outcomes of infectious keratitis after LASIK, and examine treatment strategies.

DESIGN: Retrospective study.

PARTICIPANTS: We included 107 613 patients who underwent LASIK at Clínica Baviera (Instituto Oftalmológico Europeo, Spain) from September 2002 to May 2008.

METHODS: The medical records of post-LASIK patients (204 586 eyes) were reviewed to identify cases of infectious keratitis. Incidence, risk factors, clinical course, days to diagnosis, medical and surgical treatment, and final visual outcomes were recorded.

MAIN OUTCOME MEASURES: Incidence of post-LASIK infectious keratitis, culture results, response to treatment, and visual outcome.

RESULTS: Post-LASIK infectious keratitis was diagnosed in 72 eyes from 63 patients. Onset of infection was early (within 7 days after surgery) in 62.5% of cases. Cultures were positive in 21 of 54 cases in which samples were taken. The most frequently isolated microorganism was Staphylococcus epidermidis (9 cases). Immediate flap lifting and irrigation with antibiotics was performed in 54 eyes; late flap lifting was subsequently required in 10 out of 18 cases initially treated with topical antibiotics alone. One case required flap amputation owing to flap necrosis. Final best spectacle-corrected visual acuity (BSCVA) was >or=20/20 in 38 cases (52.7%) and >or=20/40 in 67 cases (93.05%); final BSCVA was <20/40 in 5 cases (6.94%).

CONCLUSIONS: The incidence of post-LASIK infectious keratitis was 0.035% per procedure. Infectious keratitis after LASIK is a potentially vision-threatening complication. The appearance of infections in asymptomatic patients highlights the need for a proper schedule of follow-up appointments. Prompt and aggressive management of this LASIK complication with early flap lifting, scraping, culture, and irrigation with antibiotics is strongly recommended. Proper management can result in preserving useful vision.

PURPOSE: To evaluate safety and efficacy in the management of flap displacement after laser in situ keratomileusis (LASIK) and subsequent complications.
METHODS: This was a retrospective study performed using data recorded at the center’s database from October 2002 to August 2021. Efficacy and safety were both converted to bina- ry outcomes (loss of one or more lines and no change or gain in lines of visual acuity). The effects of time from surgery to complication and from complication to repair were assessed and the odds ratios and probabilities were calculated. The same procedure was applied to investigate the effect of these temporal variables on complications.
RESULTS: A total of 66 eyes with late traumatic LASIK flap displacements were studied. Efficacy remained unchanged in
48 patients (64 eyes, 75%), and safety remained unchanged in 53 patients (59 eyes, 90%). Sixty-six patients (64 eyes, 100%) achieved visual acuity values of 20/40 and 45 patients (64 eyes, 70.3%) achieved values of 20/20. The flap displacement was re- solved in the first 24 hours (SD ± 0.1 days). Surgery was per- formed in 58 patients (65 eyes, 90%). Epithelial ingrowth was the most frequent complication. Patients who underwent surgery tended not to lose lines (P = .05). The risk of developing epithelial ingrowth increases with time after LASIK surgery until traumat- ic flap displacement (odds ratio: 1.001; P < .001). The prevalence of dislocation during the study period was 0.012%.
CONCLUSIONS: Visual safety values were favorable after resolution of the flap complication. Immediate surgical man- agement leads to better visual efficacy, and the time between LASIK and trauma increases the risk of epithelial ingrowth after flap displacement.

PURPOSE: To report the atypical occurrence of diffuse lamellar keratitis (DLK) along with central toxic keratopathy (CTK) after a primary LASIK surgery.

SETTING: Baviera Clinic, Madrid, Spain.

REPORT OF CASE: A 42-year-old Caucasian female underwent uneventful bilateral LASIK surgery using the One-Use-Plus SBK microkeratome (Moria, Antony, France). Postoperative treatment consisted of topical instillation every 6 hours for a week of moxifloxacin and the combination of tobramycin/dexamethasone. Intraocular pressure remained stable during all the postoperative follow-up. Five days after surgery, the biomicroscopy showed the appearance of grade 3 DLK, in coexistence with temporary CTK, generating a significant scar and compromising the visual axis of the patient. A hyperopic defect, irregular astigmatism and an important corrected distance visual acuity (CDVA) loss was also observed. Corticosteroid treatment was immediately increased, adjusting the dose according to the evolution. Surprisingly the patient responded to topical corticosteroids and no flap lifting was required, as it is necessary in DLK grade 3.

CONCLUSIONS: In such advanced cases of DLK, flap lifting, interface cleaning and scraping seems to be only necessary to prevent flap necrosis and inflammation worsening, if there is no good initial response to the use of corticosteroids. It should be also noted that sometimes the benefit of flap lifting could be limited, as loss of tissue could occur due to corneal friability secondary to the action of collagenases.

INTRODUCCIÓN: En la literatura podemos encontrar numerosos casos de neuropatías ópticas isquémicas producidas después de la realización
de procedimientos refractivos como el LASIK (laser in situ keratomileusis).
La elevación de la presión intraocular (PIO) durante la fase de succión y tallado del lentículo se ha relacionado ampliamente con una isquemia transitoria de la retina y del nervio óptico que, junto a un mecanismo de barotrauma con compresión de células ganglionares y de la lámina cribosa, puede producir secuelas en la capa de fibras del nervio óptico. Estudios experimentales en animales sugieren que las cifras de PIO pueden elevarse incluso hasta los 360 mmHg.

CASO CLÍNICO: Presentamos el caso de una mujer de 52 años que acude a nuestra clínica con intención de corregir su hipermetropía por medio de un procedimiento quirúrgico. A la exploración la paciente muestra una agudeza visual lejana (AVL) sin corrección de 0,3 en su ojo derecho (OD) y de 0,1 en su ojo izquierdo (OI). La refracción en su OD es de +2,00 y en su OI +2,25 -0,25 x 145, llegando a alcanzar una AVL en cada ojo de 0,8 cuando este error refractivo es corregido. Presenta unas queratometría media de 41,50 y 41,75 D, y una paquimetría de 533 y 542 micras respectivamente. Se realiza un LASIK hipermetrópico sin incidencias. La AVL sin corrección postoperatoria del OD se incrementa hasta 0,8; no sucede los mismo en el OI, cuyo máximo alcanzado es de 0,3. La PIO es de 16 y 17 mmHg, respectivamente, y la refracción postquirúrgica inapreciable. La paciente refiere dificultad para conducir pero se arregla bien para el resto de actividades. Se estudia la disminución de AVL del OI y, dado que las topografías y las tomografías de coherencia óptica (OCT) son normales, se le cita para ver evolución, considerando la posibilidad de neuropatía óptica secundaria a la succión de la cirugía. La paciente no acude a las visitas sucesivas y vuelve dos años después con una AVL sin corrección en su OD de 0,3 y 0,2 en su OI. Refiere, además, visión borrosa central en AO y haber sido intervenida con éxito de una neoplasia de estómago el pasado año. Se vuelven a realizar estudios de OCT y topografía resultando normales pero además se realiza un campo visual (CV). El CV arroja un defecto centrocecal en AO por lo que la hipótesis de daño en el nervio óptico se vuelve a considerar. Tras un análisis de sangre se objetiva una falta de vitamina B12 que se asocia al déficit campimétrico y a una mala absorción debido a su proceso neoplásico. Se diagnostica una neuropatía óptica nutricional, administrando el suplemento vitamínico intramuscular de B12.

CONCLUSIONES: Los cirujanos refractivos deben de tener siempre en cuenta la posibilidad de una neuritis óptica anterior o retrobulbar después de un LASIK, bien producida por la elevación transitoria de la PIO en ciertas fases de la cirugía o bien por otras causas. Una buena anamnesis y la realización de pruebas complementarias adecuadas pueden guiarnos hacia la resolución efectiva de casos como el que presentamos.
La neuropatía óptica es una rara manifestación del déficit de vitamina B12. Se presenta como una disminución de AVL progresiva, generalmente simétrica e indolora de meses o años de evolución. Procesos que favorecen la malabsorción como cirugía digestiva (principalmente la resección de íleon), patógenos como la solitaria, el alcoholismo, el hábito
tabáquico y el veganismo se han visto implicados en el desarrollo de esta patología. Hipotetizamos con el hecho de que nuestra paciente tenía ya una malabsorción de vitamina B12 cuando acudió a nuestra clínica motivo por el cual la AVL postquirúrgica no fue lo satisfactoria que esperábamos. Tras la cirugía esta malabsorción se acentúo hasta tal punto de desarrollar una neuropatía óptica nutricional evidente. Afortunadamente, las neuropatías ópticas producidas por déficits nutricionales son reversibles si existe una rápida suplementación y actuación del oftalmólogo.
