    Nee aux Etats-Unis, la therapie familiale systemique n'a cesse d'evoluer. De nombreux courants participent a son developpement, particulierement en Europe. Dans cet ouvrage, cinq psychotherapeutes et formateurs font le point sur leurs conceptions theoriques et sur leur pratique. Therapie familiale en Europe offre ainsi l'exemple d'un systeme dynamique de cinq voix en interaction. L'ouvrage aborde des themes essentiels de la therapie familiale comme le mythe familial, les adolescents en souffrance et leur famille, le couple conjugal et le couple parental, la gestion de l'absence, ainsi que la formation des psychotherapeutes. Au-dela de ces aspects systemiques, l'ensemble des textes represente une reflexion sur l'essence meme de la psychotherapie. Cet ouvrage s'adresse en particulier aux therapeutes familiaux, mais il interessera egalement les psychotherapeutes relevant d'autres orientations s'ils sont soucieux de prendre en compte la dimension familiale de leurs patients et s'ils s'interrogent sur le sens de leur pratique.

    Massive obesity results in psychological--internal, relational, behavioural and psychosocial--handicaps due, to a great extent, to a society which encourages the occurrence of obesity but does not tolerate those who present with this condition. As the weight increases, the quality of life diminishes. Moreover, the mental performances of very obese individuals seem to be inferior to those of subjects with normal weight. Treatments which include a restrictive diet are the sources of depressive symptoms related to the chronic restriction. So far, only those patients who were operated upon, and particularly those who underwent gastroplasty, enjoy a dramatic improvement in their quality of life. As for those massively obese subjects who are not concerned with this solution, long-term supporting measures should be developed.

    To determine the manifestations of anxiety in subjects seeking anonymous HIV testing and the aptitude of professionals to perceive it, we administered a semi-structured questionnaire to 481 persons, 281 before and 200 after instructing the practitioners to better decode anxiety during the consultation. The perception of anxiety by the care-givers was analyzed using the Covi scale. The results show that subjects requesting an HIV test often report stressful relational events. Their anxiety its often underestimated by the care-givers, and aptitude to recognise the patient's emotions can be improved. Prevention and quality of counselling depend on correct perception of the emotions which prompt the subject to request a test.

    A young patient suffering from schizophrenia had intense headaches and photophobia which were induced by intra-ocular injections of mercury. The clinical diagnosis was established once foreign bodies were visualized on regular X-rays of the patients skull. The mercury intoxication in combination with the secondary irreversible lesions to the eyes necessitated a bilateral enucleation and the use of a chelating treatment with sodium-dimercapto-1-propane sulfate (DMP). Automutilation is a very rare and dramatic complication of schizophrenia. The psychiatric handling and meaning of such dramatic automutilation is discussed in this case report together with a recent review of the toxicologic treatment of mercury intoxication in humans.

    The valuation of human costs is a necessity, but this task poses many problems of method. A team made of a philosopher, a psychologist and a physician has been working with economist researchers in order to look into the meaning that the preferences announced at the time of the inquiries on human costs by QALY methods could assume. These methods are often used to obtain a valuation of the impact of a health attack on people's quality of life. The methods--in the frame of the argument assumed by the economic theory on well-being--hypothesize that people's choices depend mainly on cognitive work. The qualitative interviews show that the psychological construction process for the announced preferences largely overlap this frame. In this paper the authors hastily tackle the factors which have an effect on the preferences. They conclude that the QALY methods don't seem to be able to assess the quality of life nori to valuate the damage that the quality of life could include.

    La formazione in psicoterapia è intrinsecamente modellata dal nostro modo di pensare all'essere umano. Che cosa, nell'esistenza umana, ci espone alla (psico)patologia? Cosa può essere considerato benefico e trasformativo nello scambio terapeutico? E analogamente quale atteggiamento nella formazione, così come nella terapia, potrebbe essere più umano? Facendo riferimento al pensiero fenomenologico, gli Autori esaminano lo statuto singolare attorno al quale ruotano la psicopatologia e la terapia: il fenomeno patico. Il "patico" si riferisce alla multiforme vulnerabilità che contraddistingue la nostra condizione umana e che segna anche la nostra apertura, la nostra fondamentale sensibilità a ciò che accade nell'incontro. La dimensione patica della malattia - più che i sintomi - costituisce la realtà immediata e pregnante per il paziente. La terapia può essere definita come un movimento intersoggettivo di incontro, di comune cammino e di scambio, dove il terapeuta mette le sue conoscenze teoriche, il suo saper fare e la sua presenza al servizio della persona in cura, per tendere ad uno stare meglio con se stesso, con gli altri e nel mondo. Allo stesso modo, la formazione è chiamata a realizzare un incontro trasformativo tra formatori e studenti. La formazione in psicoterapia si baserebbe su questa iniziazione viva e pensante alla condizione umana: alla sua fondamentale natura "patica", segnata dalla possibilità sia della perdizione e dell'annientamento, che della fiducia e del compimento. Nel suo cammino, l'allievo deve poter contare non solo sull'esperienza, ma anche sulla relazione con formatori impegnati che lo sostengono e lo guidano, e sulla solidarietà attiva, sullo spirito cooperativo del gruppo dei compagni per affrontare il ‘meraviglioso' e lo ‘sconcertante' dell'incontro con il pathos.

    This study compares the clientele of a Swiss anonymous test centre with the general population tested. Information was obtained through similar questionnaires submitted to two samples of HIV-tested people aged from 17 to 45 years: the first administered in the context of a general population telephone survey (n = 245) and the second completed during face-to-face interviews of the clientele of an anonymous test centre (n = 250). The test centre sample has higher proportions of younger and single people. Attenders for anonymous testing were more likely to have acquired a new regular partner during the year preceding the interview (48.0% versus 14.4%). These differences remain when controlling for age and gender. Decision to test comes mostly from the respondent's own initiative, but suggestion from a doctor is more frequent in the general population (23.8% versus 0.8%), whereas suggestion from partner or friends is more frequent in the anonymous centre (44.4% versus 3.0%). The anonymous test centre clientele is not different from the general population tested except for the relational situation and origin of decision for testing. The test centre has become a place where the general population finds a response to a situation-specific need for HIV testing.

    Gli psicoterapeuti, fin dall'inizio della pandemia di Covid-19 e ancora oggi, sono alle prese non solo con una morte onnipresente e con un lutto in sospeso, ma anche con le questioni etiche poste agli individui, alle famiglie, alla società intera, come quelle della giustizia, della libertà, della frattura sociale, della solidarietà tra le generazioni. Devono curare, a livello esistenziale, le persone più colpite dalla pandemia il cui impatto è stato multiforme, grave e ha fatto perdere la fiducia nel futuro soprattutto tra i più giovani, perdita che ha avuto come corollario l'aumento dei comportamenti suicidari. Nell'ultimo anno i terapeuti hanno dovuto anche confrontarsi con il rifiuto della vaccinazione e l'esacerbarsi di posizioni che amplificano il senso di sfiducia e d'ingiustizia.     Immigration, a political, economic, demographic, social and ethic, as well as a medical issue, continues. Among migrants, asylum seekers, refugees and undocumented immigrants are characterised by their vulnerability, particularly related to their health status. Western physicians are more and more frequently confronted to "colorful" and often vulnerable patients. They face diseases related to international migrations; and at the same time have to integrate the differences in representations and meanings given to illness by patients of diverse origins. A bio-psychosocial and spiritual approach coupled with an evaluation of pre-migration, migration and post-migration trajectories is therefore useful for the clinician; these complementary approaches have all been integrated in the learning of cultural competencies.

    The physicians often forget to ask their patients if they would like to discuss other complaints or topics. It is sometimes quite difficult to explore the patient's complaints; while the physicians tend to focus on the immediate problem, the patients may have not only one, but several hidden agendas during a visit. In a caring relation there is a clear advantage to clarify the implicit. The search for the hidden agenda is to improve the care of i) biomedical problems ii) the social quest presented to the physicians. The sentence "Oh, by the way, doctor..." should not be only understood as an information but also as a relational expression and a reaction to the imminent separation from the physician.

    In vocational psychology there has been a growing effort to integrate different theories of career choice and development (Borgen, 1991; Hackett, Lent and Greenhaus, 1991; Savickas and Lent, 1994). As an attempt to find possible points of overlap between important variables in career choice and development this study explores the relationships between interest congruence, self-rated abilities, vocational self-efficacy and primary control, and the effect they have on job satisfaction and career decisiveness. The sample contains 284 young adults (18 to 21 years) from the German-speaking part of Switzerland who are carrying out a professional apprenticeship (of three or four years) and are about to enter the world of work. Correlational analyses reveal that the conceptually related constructs are positively correlated (interest congruence and self-rated abilities, self-efficacy and primary control).     Les migrants et les requérants s'asile qui ont survécu à des violences ou à des tortures ont expérimenté des traumatismes physiques et psychologiques qui ont laissé des traces, des plaies béantes. Une fois arrivés dans le pays d'accueil, la violence dont ils ont été l'objet ne s'arrête pas nécessairement, car l'immigration peut générer elle-même de nouvelles formes de violence qui s'adressent à des sujets rendus au départ vulnérables par leurs expériences. En effet, quand les migrants et les requérants d'asile arrivent dans le pays d'accueil, ils se heurtent souvent à une très faible tolérance de la part des population autochtones, à des contrôles policiers et administratifs, à une incertitude permanente quant au bon droit de trouver chez nous protection et sécurité.     Nous presentons «l'analyse ethique» d'une situation clinique : un jeune homme qui souffre d'un cancer colique debutant refuse categoriquement une prise en charge chirurgicale. Le medecin se trouve confronte a un conflit ethique. Les differentes options ethiques ainsi que leurs justifications respectives sont exposees. Les elements constitutifs du jugement clinique sont mis en evidente. La methodologie pour tenter de resoudre ce « conflit ethique» est presentee, telle qu'elle est mise en pratique regulierement a la Policlinique medicale universitaire.
