    Objective To assess the efficacy of tacrolimus and clobetasol propionate in the treatment of alopecia areata. Methods A clinical trial was carried out in the department of Dermatology and Venereology, Dhaka Medical College Hospital, Dhaka, Bangladesh from May 2013 to April 2014. Total sixty patients were enrolled and divided into group A and group B. Thirty of group A patients were treated with topical tacrolimus and thirty of group B patients were treated with topical clobetasol propionate. Results In group A, the duration of illness ranged from 2 months to 36 months and in group B, from 1 month to 24 months. Among the patients of group A and B, 25 (83.3%) and 27 (90%) patients improved, respectively. After 16th week of treatment, slight response in 6 (20%) and 4 (13.3%) and moderate response in 18 (59.4%) and 22 (63.2%) and marked response in 1 (3.3%) and 1 (3.3%) was seen in group A and group B, respectively.     Objective To correlate the clinical patterns of onychomycosis with the type of mycological agents in onychomycosis. Patients and methods In this cross-sectional study, 120 patients with onychomycosis, attending outpatient department, were selected by purposive type of sampling technique. They were subjected to thorough clinical examination. Nail material was collected for mycological diagnosis by microscopy and culture on dermatophyte test medium. Results Microscopy was positive in 47 (39.2%) cases, whereas culture yielded Trichophyton rubrum in 53 (44.2%) and T. mentagrophytes in 2 (1.7%) patients. Common clinical nail changes in mycologically proven cases were thickening of nail plate (68.9%), subungual hyperkeratosis (63.9%), onycholysis (55.7%), roughening of nail plate (29.5%), yellowish discoloration (29.5%) and brownish-yellow discoloration (21.3%).     Objective To compare the efficacy of combination of intralesional corticosteroid injection and cryotherapy with intralesional corticosteroid injection alone in patients of keloid. Methods A clinical trial was carried out in the department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh for duration of January 2013 to August 2013. About 30 patients (group A) were treated with intralesional triamcinolone acetonide with cryotherapy and 30 patients (group B) were treated with intralesional triamcinolone acetonide. Results Improvement rate was highest for the lesions on the chest, which was 18 (34.6%) and lowest for back 3 (5.8%).     A controlled clinical trial was conducted in the department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh to compare the side-effects of adapalene and benzoyl peroxide combination gel with adapalene monotherapy and benzoyl peroxide monotherapy in the treatment of acne vulgaris.     A cross sectional study was carried out in the department of Obstetrics and Gynae of Dhaka Medical College Hospital (DMCH). Two hundred thirty two cases who fulfilled the inclusion criteria were studied. This study aimed to assess the outcome of implementation of WHO modified partograph in labour management with special interest to evaluate the fetomaternal outcome. Among the 232 cases, engaged fetal head had more vaginal delivery, 78.8% than unengaged head. The rate of caesarean section was 26.9% in women with unengaged head vs 11.5% in women with engaged head in this study. This study showed that there was no LUCS when cases remained within alert line and outside the action line in partograph all were delivered by caesarean section.     A cross sectional study, conducted in the department of  Dermatology and Venereology, Bangabandhu Sheikh Mujib  Medical University (BSMMU) for duration of January 2009  to December 2010. Hundred twenty patients with  onychomycosis were selected by purposive type of nonprobability  sampling technique. Majority of the patients  61(150.8%) were in the age group of 21-30 years old. Mean  age of the patients were 32.8±14 years and most of the  patients were house wives 36(30.0%). Disfigurement  117(97.5%) and discomfort 89(74.2%) were more common  chief complaints of the patients. The mean duration of  disease was 20.4±15.4 months and nail fold changes were  associated with 37(30.8%) patients and more than a half  63(52.5%) of the patients had history of wet works.  Regarding the history of past illness, it was observed that  previous onychmycosis found 26(21.7%), nail trauma  26(21.7%) and immune suppression 6(5.0%). The study was conducted to compare the efficacy of the combination of topical betamethasone dipropionate and calcipotriene with betamethasone dipropionate and calcipotriene alone in the treatment of localized vitiligo. A clinical trial was carried out with the patients of vitiligo from January 2012 to August 2012.     A cross sectional study of thirty eight vitiligo patients attending at the outpatients was conducted. The result revealed generalized presentation in 53.6% cases followed by focal presentation, acro-facial and segmented presentation which were 26.3% cases, 15.8%cases and 5.3% cases. Progressive type was found in majority of cases which were 71.1% cases followed by stable case which was 39.9% cases and positive family history was found in 36.8% cases in our study. Among 38 vitiligo patients highest number of associated autoimmune disease was the thyroid abnormalities of which hypothyroidism was 15.8% and hyperthyroidism was 7.9% cases respectively. Diabetes mellitus was found in 2.6% cases, Addison’s disease and SLE were found in 2.6%case each. No autoimmune disease was found in the rest 68.5% cases.     Background: Transvaginal sonography is superior to transabdominal sonography in most cases of pelvic pathology. Objective: This study was undertaken to see the clinicalpathological characteristics of endometrial hyperplasia. Methodology: This cross sectional study was carried out from January 2007 to December 2008 for a period of two years. All suspected cases of endometrial hyperplasia were included for this study. Transvaginal sonography (TVS) performed and confirmation was done by histopathological examination. Result: A total number of 40 cases were enrolled for this study. The most common diagnosed endometrial pathology was endometrial hyperplasia which was 42.5% cases. Endometrial polyp was diagnosed in 32.5% cases. Endometrial carcinoma was in 7.5% patients and submucosal fibroid was in 10.0%. Conclusion: Endometrial hyperplasia is the most common disease among the women presented with endometrial pathology.     Animal bite is common in Bangladesh. A cross sectional  type of descriptive study was carried out at the Institute of  Public Health (IPH), Dhaka to find out the knowledge and  practices related to wound care among animal bite cases  during the month of February, 1998. A total of 200 patients  were interviewed, 55% of them came from urban area.  Maximum (65.5%) patients were teenagers and adult (above  13 yrs.) with mean age 22.15 yrs. Male and female ratio  was 80.5:19.5. Majority of cases were poor (76.5%) and  only 12% attained educational level of SSC pass or above.  In 92.5% cases dog was the biting animal, 60% of which  were stray. Only 91(45.5%) patients took immediate  measures for wound care. Of them only 11 patients knew  and washed the wound by soap and water and 3 patients did  it after getting advice from physicians. 134 (67%) patients  consulted with physicians one hour to several days after,  whereas 33% within one hour.     Endometriosis is a common cause of pelvic pain and infertility in young women. Transvaginal sonography is major means for diagnosing ovarian masses. In our study, we scanned 60 patients with endometriomata who underwent laparotomy or laparoscopy. We compared preoperative ultrasonographic diagnosis with histological reports. The sonographic criteria for the diagnosis of endometriomata were (1) cystic structure with low, homogeneous echogenicity and (2) thick cystic wall with regular margins. In 50 patients, sonography suggested an endometrioma. In 47 cases, the diagnosis was correct. The false‐positive cases were all caused by cystic teratomas with a homogeneous sonographic pattern. Ten false‐negative cases were diagnosed by ultrasonography as functional ovarian cysts (5), teratomas (3), and benign ovarian cystoma (1). Only 1 case of a 5‐mm endometrioma was demonstrated by laparoscopy but not by TVS. The sensitivity of TVS for diagnosing endometriomata was 82.4% and specificity 97.7%...

    This study looked into the relationship between the histology and clinical characteristics of burning mouth syndrome (BMS). Individuals who experienced BMS symptoms for a minimum of six months underwent clinical and histological evaluations. Preliminary assessments included recording mean age, diabetes mellitus, menopausal status, length of non-irritable and irritative salivation or symptoms, and oral hygiene. Clinical trials were conducted on the following conditions: headache, pain, non-specific health issues, taste, thirst, dry mouth, and severe menopausal symptoms. Blood tests for HBS Ag, CBC and S. Iron, S. Ferritin, S. Folic acid as well as testing for female hormones including iron, folic acid, TSH, FT3, T4, and estrogen, were among the laboratory tests. The tongue is subjected to a prospective, blind histological examination, and mucosal samples are prepared for light microscopy. The Statistical Package of Social Science (SPSS) version 23 computer statistical program (SPSS Inc. Chicago, USA) was used to compile the data that had been gathered. The tongue and oral mucosa were examined for variations in their histological and clinical characteristics using the Chi Squire and Student's t-tests; p-values less than 0.05 are regarded as statistically significant.     Lymphomatoid papulosis (LyP) is a rare cutaneous condition characterized by chronic, recurrent, and self-regressing papulonodular skin eruptions. It belongs to the spectrum of primary cutaneous CD30+ lymphoproliferative disorders, along with primary cutaneous anaplastic large cell lymphoma (primary C-ALCL) with which it shares overlapping clinical and histopathologic features. The occurrence of LyP is extremely rare, with an estimated overall prevalence rate of 1.2 to 1.9 cases per 1 million population. Patients with LyP are at an increased risk of developing cutaneous or nodal lymphoid malignancies such as classic mycosis fungoides, ALCL, and Hodgkin lymphoma.     Background Melasma is a common acquired macular hyperpigmentation which involves mostly the sun exposed areas of face and neck. Objective To assess the efficacy of the combination of 20% azelaic acid with 0.05% tretinoin cream in the treatment of melasma. Patients and methods 30 patients of melasma were treated with daily night appl ication of 20% azelaic acid and 0.05% tretinoin. Patients were followed up monthly. The disease severity assessed by SCORAD index. A 4-point scale was used to measure the level of response to treatment. Results Before treatment the respective mean SCORAD was 30.57±13.62 and 30.90±17.17 in group A and B and at the end of treatment decreased to 11.87±12.04 and 11.20±13.85, respectively ( p >0.05). Percent reduction of severity from baseline to final follow-up was 69.20±23.41 in group A and 74.77±23.30 in group B ( p =0.360).     Objective To compare the adverse effects of low-dose oral prednisolone and oral mini pulse dexamethasone in patients of vitiligo. Methods A clinical trial was carried out from January 2013 to December 2013. Total sixty patients of vitiligo were enrolled and 30 of group A patients were treated with low dose oral prednisolone (0.3 mg/kg body weight) daily and 30 of group B patients were treated with oral dexamethasone pulse therapy (10 mg per week) for 16 weeks. Results During 12 week follow-up, increased body weight, headache, dyspepsia and fatigue were more frequent in group A as compared to group B. Similarly, in group A other side effects noted were acne (33.3%), mooning of face (26.6%), striae (26.6%), hypertrichosis 13.2%, purpura (6.7%) and among the female patients, menstrual abnormality (71.4%) whereas in group B, no patient developed these problems from baseline to follow- up period (p<0.05).     A clinical trial was carried out in the department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka to evaluate the efficacy of Mometasone Furoate 0.1% cream in atopic dermatitis. Forty patients of atopic dermatitis were selected and randomized into two groups. In Group I, study patients were applied Mometasone Furoate 0.1% cream to the affected skin and in Group II, control patients were allowed to apply white vaseline to the affected skin. SCORAD index was assessed fortnightly at 2nd and 3rdvisit and final SCORAD index were calculated at the end of 4 week (3rd visit). In the study, at base line SCORAD index in case group was 49.76 ± 17.25 and in control group was 55.25 ± 13.94 (p>0.05).     The causes of polyneuropathy are many and it is important to identify them, as some of them, especially the inflammatory types are treatable. To explore the different etiological factors of polyneuropathy, this cross sectional study was carried out in the department of Neurology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Shahbag, Dhaka. Data were collected by taking medical history and clinical examination and subsequent laboratory investigations. A total of 60 subjects were included in this study and mean age of the entire patient was 42.25 year. Out of all respondents, 83.3% had muscle weakness, similar number had problems with object handling, 66.3% had muscle cramp, 63.3% had impaired standing or gait, 55.0% had distal paraesthesia, 41.7% had burning feet, 8.3% had restless legs and 5.0% had stiffness.
