This training give awareness for
control the Personnel Injuries, property damage, opening, modification, and provision of gratings.
This procedure is applicable to all structures
of the QAFCO 5 Project with ongoing construction, and completed gratings. The permit shall be signed by requesting Contractor Discipline Manager, and Contractor Area Manager. Final approval of the permit will be the Contractor Construction Manager.
During modification, the green tag shall be
shifted to red tag with warning sign (attachment 1) And only authorized workers can go up and work
(attachment 3).

Qualified Mechanical workers,
erectors and workers with training on working at height shall perform the work. Warning sign shall be placed every entrance of grating where installation is ongoing. Material shall be secured during the work. Warning barriers shall be placed around installation work on the ground. Follow permit to work for grating installation. Lack of specified materials, the grating shall be reinforced temporarily with scaffolding materials for safe use. Construction Manager

Apply disciplinary action in case of non- compliance of this procedure. Ensures monitoring by Discipline Manager/Supervisor. HSE Officers Each HSE Officer shall Monitor that this procedure will be complied when handling grating. Advise construction personnel the requirements when carrying out grating work. Ensure gratings are controlled to eliminate possible fall hazards. Sign the grating permit prior to unclamp, or modify a grating. Area Supervisor and Discipline Managers
Area and Discipline Managers
Comply with the grating procedure prior to unclamp, install, and modify a grating. Approve the application of grating permit. Close out the report when modification is completed. Make sure that grating is safe for use at all times in his area. Contractor HSE Department Maintain all records of grating permit. Review all permits Support Area and Discipline Managers Support to identify, preventive, corrective and proactive actions to rectify deviations while working on Grating. Grating to be inspected by Scaffolding inspector, Mechanical Area Manager, HSE officer. All parties shall affix their signatures on the green tag.
Tag to be Updated on weekly basis by
Scaffolding inspector, Mechanical Area Supervisor, and HSE officer.
Defected grating changed into red tag.
Grating Modification Procedure. Review its work method statement if work requires opening a Grating. Secure permit form from Contractor HSE Department.
Refer to attachment 2 :Permit Form.
Provide necessary safety controls on the perimeter
of Grating like fix barricade, warning
signboards, and if necessary work stoppage for other workgroups.
Refer to attachment 1- Signboard.
Subcontractor Discipline Supervisor, Contractor HSE Officer, Contractor Area Manager, and Construction Manager confirming closure of Grating signature . Inspected by Scaffolding Supervisor, Mechanical Supervisor, and HSE Officer. Green tag will be placed when grating evaluated as acceptable.
Close out permits shall be kept at
