We, the undersigned company, understand that the associated hardware, software and/or technical data (“products”) on
our purchase order include Commerce Control List items (CCLI) are governed by the U.S. Export Administration
Regulations (“EAR”) administered by the U.S. Department of Commerce (15 CFR Parts 730-774).
We certify that the 5 or more axes simultaneous contouring software sold and exported to our company by Centroid
Corporation under this purchase order will be used by our company for:

We have been informed by Centroid Corporation that their accompanying proprietary 5 or more axes simultaneous
contouring of software is subject to strict U.S. national security export controls. Such software will be exported to our
company under an export license exception noted in § 740.6 of the U.S. Export Administration Regulations,
TECHNOLOGY AND SOFTWARE UNDER RESTRICTION (TSR).
We certify that without authorization from the U.S. Department of Commerce, Bureau of Industry and Security we
will not:
(i) Re-export or release the technology, software or the source code for the software to a national of a country in
Country Groups D:1, E:1 and E:2 (see attached Appendix); nor will we
(ii) Export to Country Groups D:1, E:1 or E:2 the direct product of the technology or software, if such foreign
produced direct product is subject to national security controls as identified on the U.S. Commerce Control List
(CCL—Part 774 of the EAR) [See General Prohibition Three, §736.2(b)(3) of the EAR]
The obligations and requirements described in this letter of assurance shall survive the expiration or termination
of any agreement or contract between our company and Centroid Corporation. The terms of this Letter of
Assurance apply to the Company listed below which includes all subsidiaries and affiliated companies.

1. The accompanying proprietary 5 or more axes simultaneous contouring of software purchased from and
exported by Centroid Corporation located at 159 Gates Road, Howard, Pennsylvania, United States of America or
replicas thereof (“replicas” refers to items produced abroad based on physical examination of the item originally
exported, matching it in all critical design and performance parameters), will not be used in any of the activities
described in 744.2(a) of the U.S. Export Administration Regulations (EAR); and
2. Written authorization will be obtained from the U.S. Department of Commerce, Bureau of Industry and Security
prior to re-exporting the items, unless they are destined to Canada or would be eligible for export from the
United States to the new country of destination under no license required (NLR) based on country chart NP
column 1 as noted in Supplement 1 to Part 738 of the U.S. Export Administration Regulations (15 CFR Parts 730-
774).
