VESTIGE 2025
CONTRACT OF AGREEMENT
I, ____________________________ from __________ strand, hereby affirm my
commitment to take part and be a subscriber of VESTIGE 2025, The Official Yearbook of
UMDC – SENIOR HIGH SCHOOL, under the following terms and conditions:

1.​ I hereby grant authorization to Vestige 2025 to collect and utilize my personal
data to materialize the yearbook. I understand that the data I provided will be used
for the yearbook production.

2.​ I hereby grant authorization to Vestige 2025 to share my personal data and
pictures with the University of Mindanao, UM Digos College, and the External
Relations & Alumni Affairs Office (ERAAO) for publication purposes other than
yearbook production. This authorization extends to the use of my information and
images in various publications, promotional materials, and related platforms as
deemed appropriate by the aforementioned entities, and I understand that this
authorization is separate from the use of my data specifically for yearbook purposes
and may involve different forms of publication.

3.​ I shall pay Php 3,000.00 for the Yearbook Subscription and Pictorial Payment of
Batch 2024-2025.

4.​ I am entitled to a 25% rebate in excess of Php 3,000.00 from the solicited amount.

5.​ I shall return all used and unused solicitation forms on or before the deadline.
Failure to do this shall mean that I shall be charged Php 200.00 for every
unreturned solicitation form, and such charges will be included in my yearbook
fee.

6.​ I agree, understand, and follow all the steps in the yearbook subscription,
cancellation, payment, and solicitation guidelines stated in the information sheet
(google form).

7.​ I shall strictly follow all the given schedules related to the yearbook subscription,
and in case of my delay, Vestige 2025 will not be held liable for any damages.

8.​ After the yearbook’s official release date, I am aware that I only have two (2) years
to claim my yearbook. Otherwise, if unclaimed within the allotted year, I allow UM
Digos College to dispose of my yearbook.

9.​ I hereby certify that the information I provided in the Vestige 2025 Information Sheet
Google Form is complete, true, and correct.
