COME NOW, the Accused, through the undersigned counsel and unto this
Honorable Court, most respectfully state:
1. That on _______________ this case was called for hearing;
2. That however, the accused failed to appear because he was not able to
receive any notice from the Honorable Court of the said hearing date and by
reason thereof, the Honorable Court issued an Order for the issuance of a
Warrant of Arrest against him;
3. That in view of the same, the above-named Accused respectfully pray for the
indulgence of the Honorable Court that his bond posted for his provisional
liberty will not be forfeited and that the warrant of arrest issued against him be
recalled and lifted.
WHEREFORE, premises considered, it is most respectfully prayed of this
Honorable Court that the Order issued by the Honorable Court dated ___________ be
reconsidered and the warrant of arrest lifted.
