ALTERNATE BROKER AUTHORIZATION FORM

The Importer, not broker, must complete the following information and return by email to UPS Supply Chain
Solutions, Inc. Return completed form to your specialist.
Acknowledgement and signature of this document indicates that you are fully aware of and understand the
procedures of a shipment being transferred to an alternate broker for US Customs processing. This is an ad-
hoc process as Small Parcel only offers turn over service via Broker of Choice service. The Broker of Choice
service is a contractual service set up by your sales representative, setting up this service will help avoid
delays and manual processing.
UPS Alternate Broker Fee is $115.00 USD, additional $85.00 USD for IT Bond
Please read the following information carefully and select only one option (below).
If Broker does not provide release confirmation within 5 days, shipment is subject to Return to Shipper.

✔A.Option 1: Onsite Clearance, Remote Filing of Consumption Entry (RLF)


UPS will notify the alternate broker and provide import documents needed for processing.
B. Upon notification of US customs clearance, the shipment will be released, and delivery will proceed to
destination as planned. (UPS is not responsible for any delays in the clearance process incurred by the alternate
broker).
Option 2: Offsite Clearance, I.T. (61) Bond Movement to Bonded Warehouse
A. Only Immediate Transportation (61) bond movement is allowed, no T&E (62) or I.E.
B. In the space below, the importer must provide the address of the broker’s bonded warehouse that shipment should
be sent. An address correction will be applied with bonded warehouse address.
C. Alternate broker should compliantly reference this I.T. bond when moving to final destination and/or filing
consumption entry. Bond and/or Clearance should be sent to UPSSDFAlternateBrokerTeam@ups.com
within 15 days.

Importer prefers not to utilize Alternate Broker option 1 or 2, and grants UPS Customs Brokerage authorization
to submit for US Customs clearance. Importer will be responsible for all applicable duties, taxes, and fees.

A UPS Warehouse Fee is applied to any held shipment after two free business days. Thereafter, the warehouse fee will be a rate of $25
minimum plus $0.05 per lbs per calendar day after two days, $0.10 per lbs per calendar day after seven days until the package is
released. All shipments will be eligible for General Order (G.O.) warehousing by U.S. Customs on the 15th calendar day after the
shipment is imported into the United States. Any shipment picked by a G.O. Warehouse will be assigned additional warehouse fees by
U.S. Customs. These fees must be paid in full before it can be removed from the warehouse. Any shipments remaining in G.O. for 6
months may be auctioned off in accordance with the U.S. Import regulations
