AFFIDAVIT FOR CHANGE OF NAME

I, Moksha Shah daughter of Eishan Shah and wife of Shankar Menon aged 24 residing at
Mumbai, do hereby solemnly affirm and declare as under:

1. That my maiden name is Moksha Shah.

2. That I got married to Shri Shankar Menon on 20th March at Mulund, Mumbai.

3. That after marriage my name is Moksha Menon and all the records have the same name.

4. That I have obtained the marriage certificate.

5. That I state that Moksha Shah and the Moksha Menon is the name of one and the same
person and that is myself.

I am executing this declaration to be submitted to the concerned authorities for the change of
name.

I hereby state that whatever is stated herein above are true to the best of my knowledge.
