That I, am S R Chandrashekar S/o late Raje gowda aged 47 years Major, citizen of
india, #433, Near ladies Hostel, Vidyanagar Hassan, Karnataka – 573202, At
present Residing Address 2nd floor, Sarasswathipura, near ssit main gate, Tumkur.

Do hereby Solemnly affirm and state on oath declare my self before Notary as

Professional, Pan card Aadhar card, Ration card in other relevant Documents.
