PERFORMANCE MEASUREMENT

1. INTRODUCTION

1.1. HSE&S Performance Measurement is the key step in measuring the individual and
overall performance of the project.

1.2. Measurement is a key step in any management process and form, and the basis of
continual improvement. If measurement is not carried out correctly, the effectiveness of
the health, safety, environment and security (HSE&S) management system is
undermined and there is no reliable information to demonstrate managers how well the
associated risks are controlled.

1.3. Accordingly, objectives of HSE&S Performance Measurement are statistically grasping


the gist of status and exerting proactive management for improvement of relevant
standards and culture.

1.4. HSE&S Performance Measurement Procedure provides how to collect and record
HSE&S Performance data, and to report HSE&S Performance of the Project
Management Team (PMT). This Procedure shall be applied throughout the
construction and pre-commissioning phases of site works of the Rabigh Phase II
Petrochemical Project Petrochemical-1 Package (CP1) Project.

1.5. This Procedure may be periodically reviewed and amended as required to meet
specific project requirements during the construction and subsequent pre-
commissioning.

2. SCOPE

This procedure is applied to workplaces and camp accommodation premises of Rabigh Phase II
Petrochemical Project Petrochemical-1 Package (CP1).

2.1. HSE&S Performance of the project shall cover the CONTRACTORS and
Subcontractors engaged in the Project.

2.2. HSE Performance Dashboard shall be issued on a monthly basis. (Appendix 3)

HSE Engineers and HSE Advisors who are
working in RIIPED HSE Office.

CONTRACTOR : DAELIM SAUDI ARABIA CO., LTD.

Subcontractor(s) : Organization(s) that provides services to


CONTRACTOR.

4. RESPONSIBILITY

4.1. CONTRACTOR HSE Manager

4.1.1. CONTRACTOR HSE Manager is responsible for collecting, recording and


reporting recordable injuries, illnesses and incidents (including MVA, near miss,
health & environmental incidents, heavy equipment incidents etc., among
others) and recording for non-compliance of the associated government and
applicable regulations that have taken place in workplaces and camp
accommodation premises of the Project under control of the CONTRACTOR.

4.1.2. Collecting, recording and reporting of Leading Indicators, covering HSE


Inspections, Audits, Meetings, Incident Investigations, Communications, HSE
Talks, HSE Campaigns, number of closed recommendations etc. - among
others

4.1.3. The CONTRACTOR HSE Manager shall prepare Monthly HSE Performance
pertaining to the Project. The monthly performance data shall cover
CONTRACTORS’ performance and become official data from the Project.

4.1.4. Monthly HSE Performance data shall be compiled as of the last Saturday of the
month and submitted at the beginning of following week. The data shall be filled
in Monthly HSE&S Statistics Reporting Form (Appendix 2).

4.2. PMT PROJECT MANAGER

4.2.1. Assess the correctness of the HSE Data of the Project


4.2.2. Submit the Monthly HSE Performance Data to the RIIPED HSE Manager

5. RECORDING CRITERIA

5.1. Recording Criteria

5.1.1. Recording criteria and interpretation of cases relating to injury and illness shall
be in principle referred to OSHA, PRC and Saudi Aramco rules and regulations
as recordkeeping guidance for this procedure. (Appendix 1: Categorization of
Injuries/Illnesses).

5.1.2. Recordable Injury and Illness

An injury or illness must be recorded if it results in one or more of the following:

a) Death
b) Days away from work
c) Restricted work or transfer to another job
d) Medical treatment beyond first aid
e) Loss of consciousness
f) A significant injury or illness diagnosed by a physician or other licensed
health care professional

5.1.3. Recordable Incident and data

The following incidents and data must be recorded.


a) First aid incident (FA)
b) Near miss case (NM)
c) Motor Vehicle Accident (MVA)
d) Property / Equipment damage
e) Environmental incident
f) Fire Incident
g) Release/Leaks Incidents
h) Heavy Equipment Incidents
i) Traffic Violations
j) Solid/Hazardous Waste On-site/Off-site Disposal
k) Over exposure to a health hazard(s)
l) Non-compliance to the applicable regulations

PERFORMANCE MEASUREMENT
5.1.4. There may be a case that a motor vehicle accident (MVA) took place and
consequently a person slightly injured and/or property damaged. In such case
one MVA and FA and/or property damage shall be recorded so that analysis
can correctly address on real causes.

5.1.5. HSE&S Leading and Lagging Indicators must be recorded, tracked and
reflected in the Monthly HSE Performance Report (Dashboard) such as Number
of HSE Audits, HSE Talks, HSE Inspection and HSE Walkthrough and other
criteria mentioned in Section 4.1.2 above.

5.2. Lost Time Injury Rate: LTIR

This Incident rate is based upon the total number of Lost Time Injury cases which
resulted in death plus the number of Lost Time Incident per 200,000 man-hours
worked (which is equivalent to 100 persons working in a calendar year - per
OSHA), but exclude Restricted Work Day Case (RWC) and Medical Treatment
Case (MTC);

LTIR = (Fatality + No. of Lost Time Incidents) x 200,000


Total Man-Hours

5.3. Total Recordable Incident Rate: TRIR

This Incidence rate is based upon the total number of Recordable cases per
200,000 man-hour, any work-related injury and illness case requiring more than
first aid that fall into any one of the following:

 Fatality
 Lost Time Incident
 Restricted Workday Case
 Medical treatment (other than first aid);

Total Recordable Cases are the sum of Lost Time Incident (LTI), Restricted Work
Case (RWC), Medical Treatment Case (MTC) and Occupational Illnesses but
excluding First Aid Case and Near miss Case:

TRIR = (Fatality + LTI + RWC + MTC) x 200,000


Total Man-Hours

The type of Recordable Incident will be decided by joint agreement between Site
HSE Manager and the Physician who administered the victim.

5.4. Severity Rate: SR

This Incident rate is based upon the total number of Lost Workdays per 200,000
man-hours:

6. HSE&S PERFORMANCE DASHBOARD

6.1. Leading Performance Indicators

6.1.1. HSE&S Performance shall be measured by a combination of Leading and


Lagging Indicators.

6.1.2. Leading Indicators include (but not limited to), HSE Inspections, Audits,
Meetings, number of completed Incident Investigations, HSE&S
Communications (HSE Incident Alerts, HSE Bulletin, HSE Incident Information),
HSE Talks, HSE Campaigns, number of closed HSE recommendations,
Behaviour Safety Observations, number of near miss raised, etc.

6.2. Lagging Performance Indicators

6.2.1. HSE&S Performance shall be measured by a combination of Leading and


Lagging Indicators.

6.2.2. Lagging Indicators include (but not limited to), incidents, injuries, motor vehicle
accidents, environmental incidents, incidents related to health (such as food
poisoning, outbreak of communicable disease), LTI Rate, Severity Rate, MVA
Rate, security violations, waste disposal, etc.

6.3. Development of HSE&S Performance Dashboard

6.3.1. The development of the HSE&S Performance Dashboard will be performed by


the CONTRACTOR HSE Department

6.3.2. CONTRACTOR HSE Manager shall provide data related their respective
Package leading and lagging indicators to RIIPED HSE organization.

6.3.3. CONTRACTOR HSE organization shall record the data in a software program
(MS Access or others, as appropriate).

6.3.4. CONTRACTOR HSE organization shall develop the Project HSE&S


Performance Statistical Data as well as the Dashboard from the software
program. Such data / dashboard shall be submitted to PMT and also for the
RIIPED DO HSE for record.

6.4. Periodicity of Report and Distribution

The Periodicity of Report and Distribution will be set by CONTRACTOR HSE


Organization.

6.4.1. Project HSE&S Performance Statistical Data as well as the Dashboard shall
be prepared by CONTRACTOR HSE organization at monthly intervals (for
each month) and annually – as a minimum.

6.4.2. Depending upon the need, the data and dashboards may be prepared at
additional intervals.
APPENDIX 1: Categorization of Injuries

HSE&S incidents shall be categorized in accordance with OSHA, PRC and Saudi Aramco.

Fatalities

A fatality shall mean an incident causing death regardless of the time between the injury and
death.

Lost Time Injuries

A lost time injury (LTI) shall be an injury, including a fatality, that results in a lost work day, but
not including the day of injury, during which the employee would have worked but could not do
so, that is could not perform all or part of his normal assignment during all or any part of the
workday or shift, because of the occupational injury.

PERFORMANCE MEASUREMENT

Recordable Injury

A recordable injury is an injury, including a lost time injury, which result in transfer to another job,
or termination of employment, or require medical treatment (other than first aid), or involve loss
of consciousness or restriction of work or motion. Typical medical treatment includes:

 Treatment of infection,
 Application of antiseptics during second or subsequent visit to medical personnel,
 Treatment of second or third degree burns,
 Application of sutures (stitches),
 Application of butterfly adhesive dressings or sterile strips in lieu of sutures,
 Removal of foreign bodies embodied in the eye,
 Removal of foreign bodies from wound if procedure is complicated because of depth of
embedment, size or location,
 Use of prescription medication (except single dose administered on first visit for minor
injury or discomfort),
 Use of hot or cold soaking therapy during second or subsequent visit to medical
personnel,
 Application of hot or cold compresses during second or subsequent visits to medical
personnel,

 Cutting away dead skin (surgical debridement),


 Application of heat therapy during second or subsequent visit to medical personnel,
 Use of whirlpool bath therapy during second or third visit to medical personnel,
 Positive X-ray diagnosis (fractures, broken bones, etc.),
 Admission to a hospital or equivalent medical facility for treatment.

First Aid Injury/Injury/Illnesses

 Using a non-prescription medication at non-prescription strength


 Administering tetanus immunization
 Cleaning, flushing or soaking wounds on the surface of the skin
 Using wound coverings such as bandages, Band-Aids, gauze pads, etc; or using butterfly
bandages or steri-strips.
 Using hot or cold therapy treatments.
 Using any non-rigid means of support, such as elastic bandages, wraps, non-rigid back
belts, etc.
 Using temporary immobilization devices while transporting an accident victim (e.g.
splints, slings, neck collars, back boards, etc.)
 Drilling of a fingernail or toenail to relive pressure, or draining fluid from a blister.
 Using eye patches.
 Removing foreign bodies from the eye using only irrigation or a cotton swab.
 Removing splinters or foreign material from areas other than the eye by irrigation,
tweezers, cotton swabs or other simple means.
