DESCRIPTION OF THE PATIENT

The patient is29 years old male who is a soldier. His level of personal hygiene is fairly good and
neatly dressed. The patient engaged the nurse0in a cooperative, friendly and polite manner. His
speech was spontaneous with normal rate, rhythm and tone. His reaction to the questions that
were asked by the Nurse was normal.

DESCRIPTION OF ENVIRONMENT

There were two places that the conversations took place in, the first one is in the garden where
the patient freshen up and in a quiet room where the interview had started. The conversation
was supportive all the way around. There were minimal distractions in that area, there were a
few people that passed by, but the patient did not get distracted when the nurse were talking
to him.

OBJECTIVES

-Establish rapport and a trusting relationship with the client to encourage the patient to
verbalize feelings and the reason behind them.
-Express his feelings after interaction.

EVALUATION

The objectives set by the student nurse were all carried out. At first, the student nurse was very
anxious to talk with the patient. But, after a while the student nurse felt very comfortable going
into the interview with him. The patient engaged in a cooperative, friendly and polite manner.
The patient was responsive to all the questions asked. He is able to verbalize what he really
feels. The patient was able to recall all the past events of his life.

The patient was quiet and goal directed. He comprehends fairly well. The patient admits that he
hears words mumbling and seeing things. He also admits that the voices that he hears were
whispered to him to harm the nurse.

DESCRIPTION OF ENVIRONMENT

In working place, the conversation took place in a quiet room. The light and temperature were
appropriate for the conversation and it was not uncomfortable; which made this part of the
conversation supportive.

By the end of the working phase the patient will:

-To make a realistic plan in accordance with the existing capabilities at him.

goal directed. The patient can still recall the past events that took place in his life and the
reason that he comes and go in the institution. He is also taking all of his prescribed medicines.
He also stated that he will do anything so that he can be reunited again with his family.

The patient‘s level of hygiene is fairly good and neatly dressed. His mood was euthymic. The
reason why the patient was in the institution is because he burned their store and he claimed
that he heard words mumbling and seeing things which he knows that is not true.

DESCRIPTION OF ENVIRONMENT

The last part of the conversation took place in a four sided room. There were no distraction and
the light and temperature were comfortable and did not cause a distraction, which made the
conversation supportive.

- View the student nurse as honest, open and concerned about his welfare.
