Blk. 25 Lot 42 Purok 3 Bonifacio St., Upper Bicutan, Taguig City, Philippines, do hereby name, constitute and appoint JAYMAR DATULAYTA MANDUYOG, of legal age, Filipino, and a resident of Al-Ittihad St. Bagua II, Cotabato City, to be my true and lawful attorney-in-fact, for me and in my name, place and stead, and perform any and all acts required as my authorized representative:
1. To transact, process, claim and fix the correction of my name in my
birth certificate, with the Philippine Statistics Authority (PSA), Cotabato City; and
2. To sign, pay, execute, deliver, and claim any and all documents or instruments necessary and to perform any and all acts necessary and required, or impliedly included, for the accomplishment and performance of the powers herein above mentioned.
