GUIDELINES FOR PROVIDING SPECIMEN SIGNATURE ON PLAIN PAPER

Please follow the quick step-by-step guide listed below:

1. Kindly use a black or blue ball point pen to fill your Signature Specimen Card.

2. Mention the following text on top of the paper:

SPECIMEN SIGNATURE
MEEZAN ROSHAN DIGITAL ACCOUNT

3. You shall then write your full name in BLOCK LETTERS as mentioned in your ID document.
Note: Your name in the Specimen Signature Card should match with your name provided in the
Account Opening Form.

4. Please sign your signature as per your ID document.

5. Scan or capture an image of your Signature Specimen Card and upload it with the other required
documents for opening your Meezan Roshan Digital Account.

customer name and signature mentioned here are fictional and therefore have been produced for
