KEEP THIS RECORD FOR AUDIT PURPOSES


DO 198-18, Sec 8: PPE shall be provided to the workers, free of charge.
All PPE shall be of the appropriate type as tested and approved by the DOLE based on its standards and/or other means of verification.
All PPE must be of appropriate size, weight and type to specific workers exposed to the hazard.
Issuance of PPE shall be supplemented by training on application, use, handling, cleaning and maintenance of the PPE by the Safety Officer.
