PETITION
PETITIONER, unto this Honorable Court, most
respectfully states that:
1. She is of legal age and a Filipino citizen;
2. She is presently residing at 276 T. Candido Pesa St.,
Brgy. Talisay, Lipa City, Province of Batangas;
3. She has satisfactorily completed the required course
in Law at Centro Escolar University, Gil Puyat Avenue,
Makati City in March, 2016;
4. She is a member of the Philippine Bar, having been
admitted on May 16, 2022 as evidenced by a her
Certificate of Membership. This is attached hereto as
Annex “A” and made an integral part of this Petition;
5. Petitioner is a private practitioner with office address
located at Lipa City Hall, Brgy. Marawoy, Lipa City.
A copy of the sketch to the Petitioner’s law office is
attached as Annex “B”.
6. She is a member of the Philippine Bar in good
standing with Clearances from the Office of the Bar
Confidant of the Supreme Court and the Integrated
Bar of the Philippines (IBP). The Certificate of no
pending administrative case from the Office of the
Bar Confidant is attached hereto as Annex “C”,
while the clearance from the IBP is attached as
Annex “D”.
7. The undersigned has not been charged nor convicted
in the first instance of any crime involving Moral
Turpitude as evidenced by a clearance issued by the
National Bureau of Investigation (NBI) attached herein
as Annex “E”;
8. She desires to be commissioned as a Notary Public for
and in the City of Lipa and possesses all the
qualifications and none of the disqualifications for said
office;
9. In compliance with the requirements of the Honorable
Court for Notarial Commission, attached hereto are the
following annexes and made an integral part of this
petition:
a. Three specimen official signatures as
Annex “F”;
b. Three (3) pieces of 2x2 picture as Annex
“G”;
c. Copy of IBP Local Chapter Certificate as
Annex “H”;
d. Copy of Professional Tax Receipt as Annex
“I”.
10. Furthermore, in accordance with the Rules,
Petitioner will inform the Executive Judge within ten
(10) days from the date of any change in the
previously given office address and shall submit his
monthly report to the Office of the Clerk of Court of
Lipa.
PRAYER
WHEREFORE, premises considered, it is most
respectfully prayed of this Honorable Court that:
1. The Petition for Commission as Notary Public
filed by the undersigned be acted favourably;
2. Petitioner be commissioned as a Notary Public
in the City of Lipa.
