DOCUMENTATION REPORT FOR
MODULE 6 ASSIGNMENT
I. INTRODUCTION
Mayor Santiago Garcia Rabat (MSGMS) plays a critical role in bringing the DepEd's goal and vision to
fruition. Because of the poor MPS in School Monitoring and Evaluation, it is suggested that
instructional leadership be strengthened. The DepEd Order 42, 2016, which institutionalizes
instructional planning as a crucial aspect of the teaching and learning process, is one of the
foundations for strengthening instruction.
As such, lesson preparation has specific functions to fulfill that are critical to effective teaching. It
offers the teacher more confidence and independence in the classroom. The educator who has
meticulously arranged his lesson enters the classroom relaxed and ready to begin. It enables the
teacher to understand the most desirable types of teaching techniques as well as suitable progress
and check assessments for evaluating instructional outcomes.
In short, the teaching-learning process require instructional planning. Effective teachers, according
to research, arrange and plan their lessons. The delivery of teaching and learning in schools relies
heavily on planning. Based on this premise, the school head held a School LAC Session on facilitating
the teaching-learning process with the integration of the lesson planning development.
Per instructions, I sought the permission of my Schools Division Superintendent (SDS) through the
recommendation of the Public Schools District Supervisor (PSDS) to announce a shortened period to
conduct my meeting/INSET session with my teachers on November 25, 2021.
A total of 12 Teachers attended the meeting. I started the session with preliminaries and discussed
the following:
1. Understanding the Facilitative Teaching-Learning Process
2. Towards Effective Teaching-Learning
3. Enhancing Questioning and Active Learning Skills for Effective Teaching
Attached as Annexes “A,” “B,” and “C” are copies of the Memorandum requiring the attendance of
the Teachers in the INSET/Meeting on November 25, 2021, Attendance and Minutes of the Meeting,
and PPT presentation, respectively.

II. WORKSHOP/DEVELOPMENT OF LESSON PLAN
With their agreement, the subject chosen as an example will be Grade 3 English. A total of 12 out of
the 25 Teachers who attended the INSET/Meeting were tasked to develop the Lesson Plan. Other
teachers were still on deck for their schedules in lesson plan development adhering to the on-site and
work from home arrangement. Teachers were focused on preparing for their assessment, evaluating
students' modules and tests, conducting home visits for reading intervention programs, and
attending webinars. Due to restrictions of the COVID-19 pandemic, time conflict is a major issue, as is
the present alternative work arrangement.
Immediately after, the completed Lesson Plan was submitted to me for review. Upon incorporation
of my comments/recommendations, the teachers have agreed to implement the Lesson Plan on the
first week of December 2021.
Attached as Annexes “C,” “D,” and “E” are copies of the “Pictures” during the Workshop, the Lesson
Plan on the subject English 3, and the Lesson Plan with my comments/recommendations.
III. IMPLEMENTATION OF THE LESSON PLAN
As agreed, a total of 3 teachers were assigned to take care of the implementation of the Lesson Plan
on the subject. A total of 0 student of grade N/A section N/A participated in the implementation of
the Lesson Plan. Peer-teaching was utilized.
NOTE: NO STUDENTS were involved in the implementation of the Lesson Plan as the school is
employing Modular Print Distance Learning Delivery Modality.
Attached as Annex “F” are copies of the Pictures taken during the implementation of the Lesson
Plan.
IV. POST OBSERVATION/IMPLEMENTATION MEETING
The post observation meeting was conducted on December 3, 2021. Peer demonstration teaching
was done because the school is implementing a Modular Print Distance Learning Delivery Modality.
The following is a summary of our discussion.
1. Teachers are encouraged to vary the activities in the presentation of the lesson.
2. Teachers are oriented on how to be a facilitative teacher.
3. Primary levels (k-3) were advised to provide manipulative and interactive activities so
that they may participate actively in activities and enhance their psychomotor abilities.
4. Teachers in the higher grades were encouraged to create an online group chat where
students could seek for help, for inquiry and urgent announcements.

V. REFLECTION
In approximately 500 words, write a reflection on how the activities were conducted in this module assignment. In this short
reflection, make sure you answer the following questions:
What worked and what did not work when your teachers implemented their respective lesson plans?
What feedback did you provide to your teachers? Give examples.
What other support did you extend to your teachers on their use of enabling questions and active learning strategies?
Teachers are teachers where they are ready to make and accept changes. They are willing to
adapt whatever improvement and changes in teaching strategies, considering that in the field of
educating children, to adapt change is a pre-requisite. Sometimes the plans of teachers’ activities
were met and sometimes not.
Teachers’ characteristics and behavior is very important. But honestly speaking, it is very
difficult to implement on how to be a good and a facilitative teacher. Teachers aims and goals is to
give the best for the students. That they will produce a competent and productive student that can
compete with other people in terms of education.
As a school head, I support my teachers by giving them materials needed, coaching them,
and mentoring them. I call for a SLAC session to help each other because I strongly believe that more
heads are better than one. I encourage teachers to read educational journals that help them develop
their communication skills and gain knowledge on teaching and learning process. Through reading
keeping abreast with print and digital breakthroughs in education, we will be updated on new ideas,
techniques, and strategies in teaching-learning process.
Every child is unique in all ways. Educators must study the learning abilities of the learners
and ready to give motivation that best fit to the needs of every learner. Our characteristics and
behaviors as teachers are deemed necessary. To meet our objectives. We must consider the
individual differences of our learners. Teachers must adjust according to the voice of the learners.
Our understanding, patience, love, and concern are the best weapon to help absorb the interest to
learn among learners.
Every school head must be sensitive to teachers’ and learners’ needs. School head plays a
big role in giving full support on how to achieve high academic performance, see to it that objectives
are congruent to the lesson, provide instructional leadership and influence stakeholders to give
support in our educational system.
For a teacher, having a supportive leader can make all the difference. One of a school head's
primary responsibilities is to provide ongoing, collaborative teacher support and to gradually build a
foundation of trust to get to know each teacher's strengths and shortcomings.
No teacher is perfect, but there is always opportunity for development, which is why a
genuine review is so important. Giving feedback is an essential component of any teacher's career.
Teachers can grow and expand their profession by identifying their performance weaknesses and
strengths, which leads to them improving their teaching approaches for the benefit of their
students.
