I, MISS BULU DAS, aged about 41 years, D/O of Lt. Sarda Chandra Das
(Father) & Lt. Kalpana Das (Mother), by religion Hindu, a resident of Gooda Pukhuri
Road, Sripuria, P.O.,P.S, & District- Tinsukia Assam do hereby solemnly affirm and
declare on oath as follows: -

1. That my Mother Kalpana Das (Mother), (since deceased) expired on 30 th


November 1998, due to illness at Tinsukia, Assam.

2. That my Father Late Sarda Chandra & my Grandparents are pre-deceased.

3. That at the time of death my deceased Mother left the following legal heirs:-:-

death.

6. That I swear this affidavit as a token of proof of the fact that my Mother died on
30/11/1998 and leaving the aforesaid legal heirs only.

7. That this Affidavit is to be submitted before the any authority concerned as a token
of evidence regarding the proof of aforesaid facts.

8. That the forgoing statements are true to the best of my knowledge, belief and
information and I conceal nothing about the matter.
