LPD/CSU Tanajib Area Construction Safety Team conducted a regular field safety inspection at Sinopec EPC 17 TGPPD. We would like to
bring to your attention some deficiencies that we noticed during the inspection. These observations were discussed/communicated thru
email with Project Management Field Compliance Coordinator Darius Jose. We appreciate your immediate action to address them in a
timely manner.

Findings & Recommendations

Finding 1
At pipeline km 14, the CSE supervisor Ashad Uzzaman, ID no. 2438497188 did not review and communicate (as part of his
responsibilities) the CSE plan to all involve personnel prior to allowing the entry to confined space.
Recommendation 1
Status: Approved Submittal Assigned To: Nader Mazyad (MAZYNM0A) Due Date:
CSESs (e.g., Supervising Operator, Work Permit Issuer) shall review the confined space entry plan that is unique to the specific confined
space and communicate the confined space entry plan (e.g., specific confined space hazards, precautions, responsibilities, emergency
procedures) to entrants and standby men as per CSM I-6 Confined Spaces, paragraph 6.4.1. A. and B.
