Re:   ASSIGNMENT ON THE CONDUCT OF EARLY REGISTRATION FOR SCHOOL
      YEAR 2022-2023
      1. In consonance to DepEd Memorandum No. 017, s. 2022, on the conduct of early
         registration for school year 2022-2023, from March 25 to April 30, 2022. The school
         hereby created committee to undertake the said activity to wit.
         For School Enrollment Chair
         BAUTISTA P. ACQUIATE: Grade 12 Enrollment Chair
         KEVIN E. ESTUDILLO
         JEZREL B. MANLANGIT: Grade 11 Enrollment Chair
         THOMAS M. CABENIAN
         1. Review all Summary Matrix submitted by advisers, check for accuracy/completeness
         2. Prepare a Summary Matrix with totality for all items/questions of all sections
         3. Submit the Accomplished Summary Matrix (Grade level) to School Enrollment Focal
            Person (SEFP)
         For School Enrollment Focal Person (SEFP)

         1. Review all Grade Level Summary Matrix submitted by GLEC, check for
            accuracy/completeness.
         2. Prepare a Summary Matrix with totality for all items/questions of all Grade Levels.
         3. Submit the Accomplished Summary Matrix (School level) to School Head for review
            and approval and then to LIS System Administrator.
     For LIS System Administrator
     DERICK R. REBAY
     a. Review the School Level Summary Matrix validate the correctness of enrollment
        count vis-a-vis the number of respondents.
     b. Login to LIS and click the QC Folder available in the Dashboard.
     c. Input total count for each table as appeared in the Summary Matrix. May use the
        assigned code as appropriate for easy reference.
2. For more details on the rules, duties, and functions of the above concerned teachers as
   committee chair and member kindly browse to the following attachment:

    b. Annex instructions
    c. Enrolment Survey form
3. Your services rendered on the conduct of this activity, especially during Saturdays will be
   entitled to service credits. If done remotely in areas under alert level 3 to 5, precautionary
   measure must be observed, physical distancing, health and safety protocols will be strictly
   observed.
4. Further, each identified person is intitled to attend a meeting at the principal’s office at
   1:00 pm today (March 25, 2022).
