        AFFIDAVIT OF LOSS
  
I, LEAH TUMABCAO y NOVENO, single, of legal age, Filipino, residing at
Barangay Carisquis, Luna, La Union, Philippines, after being duly sworn to in accordance
with law, do hereby depose and say that:
 

1. I finished my college education at the LORMA COLLEGES and graduated


thereat in March 2012;

2. As such, I was issued a diploma and a Transcript of Records by the said school;

3. Sometime in 2019, I submitted the original copy of my diploma and transcript of


records to Philippine Coastguard as pre-employment requirements;

4. Recently, I went to the Philippine Coastguard to retrieve the original copies of the
aforementioned documents but the same cannot be located by the said
government agency;

5. Efforts were exerted to locate and recover the said collegiate credentials, but in


spite of diligent search, the same cannot be located and is now beyond recovery;

6. In the event that the said documents should hereafter be found or recovered, I
undertake to forward or surrender the same to the said school, for its appropriate
disposal;

7. I am executing this Affidavit to attest to the truth of the foregoing and for


whatever legal purpose it may serve. 
 IN WITNESS WHEREOF, I have hereunto affixed my signature this February 11,
2022 at San Fernando City, La Union, Philippines.
