AFFIDAVIT OF SUPPORT AND CONSENT


OF TRAVEL OF A MINOR
We, spouses, ________________ and _____________________, both of legal age,
both Filipinos, and residents of _______________________, after having been
duly sworn to and in accordance with law, hereby depose and say:
1. That we are the legitimate parents of ________________, nine (9) years
old and a grade school student of Ateneo de Davao University.
Appended as Annex A is a legible copy of her Birth Certificate to
prove this fact;
2. That we are giving our full consent to our daughter, ______________, to
travel to Manila to participate in the Rizal Football Association
Tournament. On February 7, 2015, she will depart Davao for Manila on
Cebu Pacific Flight #5J-964 wherein she will be accompanied by her
coach, ______________________, with residential address at Mangga St.,
cor. Luisa, Juna Subdivision, Matina Davao City;
3. She will return to Davao from Manila on Cebu Pacific Flight #5J-977 on
February 9, 2015 wherein she will be accompanied by her teammates
father,
______________,
with
residential
address
at
________________________;
4. That for this purpose, we have sufficient and adequate financial
capacity to support and defray the said travel and do hereby undertake
to finance her trip to Manila from the application fees, airline tickets,
board and lodging, pocket money and such other expenses that she
may incur so that she would not be a burden at her place of
destination at any time during her trip to Manila;
5. That we are executing this affidavit to attest to the truth of the
foregoing facts and to apprise the proper authorities concerned of our
full consent for our daughter to travel to Manila and back to Davao,
unaccompanied by us, her parents, but by the persons aforecited in
paragraphs 2 and 3, respectively.
IN WITNESS WHEREOF, We have hereunto set our hands this 4 th day of
February, 2015 at Davao City, Philippines.
