Disclosure
Kwik Trip may obtain or cause to be prepared a consumer and/or investigative consumer report
("Report") concerning you for employment purposes as part of its screening and hiring process, or, if
applicable, during your employment with Kwik Trip. The nature and scope of this investigation will
include a number of sources including criminal convictions, motor vehicle records, and other reports. A
Report, unless prohibited by applicable law, may include information bearing on your character, general
reputation, or personal characteristics. Information relating to your performance and experience, along
with reasons for termination of past employment from previous employers, may also be obtained.
Information may be obtained from various Federal, State, County and other agencies that maintain
records concerning your past activities relating to; driving, criminal, education, and other experiences
and, for investigative consumer reports, from personal interviews with your friends, neighbors or
associates (most commonly from your prior employers and references).
If an investigative consumer report is obtained or caused to be prepared, Kwik Trip will provide you
the nature and scope of the investigation requested upon a timely written request by you to Kwik Trip's
H uman Resources department.
Drug and Alcohol
Disclosure
I understand that drug and alcohol testing is a requirement for the position I am seeking. I further understand
that any offer of, or continuation of employment, will be contingent upon successful completion of a drug test.
Additionally, should the company obtain a positive or invalid drug screen result, Kwik Trip, Inc. has the right to
take disciplinary action up to and including termination.

Authorization
I hereby authorize Kwik Trip to obtain from a consumer reporting agency a consumer and/or an
investigative consumer report regarding me during the selection process, and, if applicable and as allowed
by law, during my employment with Kwik Trip without asking me for my authorization again.
I understand that any offer of, or continuation of employment will be contingent upon successful completion
of a satisfactory background investigation, where applicable. I further understand that should the company
become aware of information withheld during the selection process, Kwik Trip, Inc. has the right to take
disciplinary action up to and including termination.
A copy of the Company’s Drug Free Workplace Policy is below. I have reviewed a copy of Kwik Trip’s Drug
and Alcohol Testing Policy (“Policy”). Consistent with applicable law and the Policy, I consent to the
disclosure of the drug and alcohol test results to Kwik Trip, and/or its agents, including, but not limited to, a
Medical Review Officer.
By signing below, I further acknowledge that I have reviewed the Policy, I have been provided an opportunity
to ask questions about my rights under the Policy, and that I may be required to take a Drug and Alcohol Test
consistent with the Policy.
