Transition Mapping Guide for Individuals

This document provides a transition mapping guide to help trans individuals decide which aspects of transition are relevant and make a plan to achieve their goals. It suggests considering transition goals in physical, social, and legal categories. Sample goals are listed in each category to help people identify their priorities and how earlier goals may enable later ones. The guide prompts users to list their own transition goals and steps to reach them, then provides questions to help solidify initial steps.

TRANSITION

MAPPING
GUIDE
Finding the path for our own transition can be
overwhelming. This guide is meant to be a living
document that can change as your goals and needs
do. It can help you decide what aspects of
transition are relevant to you, serve as a guideline
to map your transition, and help you make plans to
achieve your transition related goals. It’s not
perfect and won’t encapsulate the entirety of what
transition can mean or look like, but it’s meant to be
broad enough that you can adapt it to fit your
needs.

On the next page, list out the goals that are important to you, the goal of
the map is to help you see what's important to you and how those goals
might be connected to one another. For example, top surgery might be
most pressing to you, but you may find it easier to do if you start HRT
first (if that's already one of your goals)

Which feels most pressing?

If these all still feel far out, what are some things you can do to affirm yourself in
the meantime?

What are your first steps?

What can you get started on this week?
