Select Post Applied for as House Surgeon 6. Enter Your Name, Parentage , DOB, Address at desired text boxes. 7. Select Others in DEPARTMENT field for House Job 8. Enter 500 as an amount to be paid. 9. Fill other required information and click submit for payment. 10.
