SCHOOL FRONTLINE SERVICES
Enrolment (Kinder/Elementary)

Release of School Form 9 & 10
Filing of Complaints/Appeals/Motions for Reconsiderations
Request Permit to use classrooms, furniture’s and other physical facilities in school

ENROLLMENT-ELEMENTARY
Who may avail of the Service?
Kinder age - (5- years old)
Elementary age - (6-11 years old)
Schedule of Availability of Service:
1. Kinder and Elementary: two (2) weeks before the start of the academic school year
Requirements for Enrollees:
Kinder and Elementary
Report card for Grade II-VI pupils/students
Birth Certificate-PSA (original/photocopy) for Kinder and Grade I pupils
Transferees
Certificate of good moral character
School Forms 9 and 10
Personnel in charge------Adviser

REQUEST A COPY OF CERTIFICATE OF


ENROLMENT, GOOD MORAL CHARACTER,
SCHOOL FORMS 9 & 10
Make a letter request to the concerned personnel in charge for the release of the
documents—School Guidance Coordinator/Teacher Adviser
REQUEST PERMIT TO USE SCHOOL
FACILITIES
Submit letter request to the School Property Custodian providing the purpose and urgency
of the usage then to be approved by the School Head.

FILING OF COMPLAINTS
Schedule of availability of service:
Monday-Friday
8:00 am – 5:00 pm NO NOON BREAK

Who may Avail the Service?


Any person may report complaint against teaching and non-teaching personnel of the
Schools.
Send letter request in order to resolve/mediate the conflict immediately
Please see/contact the personnel in charge- School Guidance Coordinator and the
School Grievance Committee
