The Deponent solemly affirms as follows:-

1. That Shri. Late. _________ was my Husband and He died on 07 th March, 2004.

2. That the deceased has left behind the following heirs (Widow/Wodower/mother/Son/Daughter/
Widow of predeceased son/ Children of Predeceased daughter/Children of predeceased son
etc.) including myselfy under ther Hindu Succession Act, and their names along with their
relationship with deceased, their ages and their addresses are given below:-

conceals nothing which is relevant to the above matter.
