AFFIDAVIT OF ADMISSION OF PATERNITY

I, ARCHIBALD GALOPE, of legal age, residing at Brgy. Catmonon, Ezperanza,


Agusan del Sur, after having sworn to in accordance with law hereby depose and say:

1. That sometime in 2014, I had a relationship with NERIZA PASTOR BATOY, of

legal age and a resident of Brgy. Catmonon, Ezperanza, Agusan del Sur;

2. That as a result of such relationship, she conceived a child sometime in July 2017,

and is expected to give birth on March 2018;

3. That I am the biological father of the said child;

4. That I am not legally married to NERIZA PASTOR BATOY, the biological mother of

my unborn child;

5. That I hereby admit under oath my paternity of the unborn child and obligate myself

to give sufficient financial support for his/her basic needs, education, health care

and his/her other necessities until reaching the age of majority;

6. That when my financial condition become stable and it will be practicable to

establish a conjugal and family life, I will undertake to marry NERIZA PASTOR

BATOY;

7. That I am executing this affidavit to attest to the truth of the foregoing facts and for

whatever legal purpose it may serve.
