JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


(FOR DELAYED REGISTRATION OF BIRTH)

We, Mrs. MYRNA A. RAMOS, Filipino, of legal age married and a resident of Sitio
Mambog Brgy. San Jose, Baras, Rizal and ADELINA B. CALDERON, Filipino, of legal
age and a resident of 57 L. Wood St., Brgy. San Isidro, Taytay, Rizal, after having been
duly sworn to in accordance with law, do hereby depose and state that:

1. We personally know LEON S. LIGOY who was born on March 9, 1945 in the
Municipality of Baras, Province of Rizal to his biological parents, Sotero Ligoy
and Antonia Ligoy y Sandy, the undersigned affiants being personal
acquaintances and former neighbors of the family of Sotero Ligoy;

2. The birth certificate of said Leon S. Ligoy was not immediately registered with
the Office of the Civil Registrar of Baras, Rizal primarily because of the
inadvertence of the parents and/or their lack of knowledge as regards the
system of registration during that time, hence this Joint Affidavit;

3. The birth certificate of the said child is not registered with any of the Local /
Municipal / City Registrar and that the enclosed PSA certificate indicate that
the Philippine Statistics Authority likewise has no record of the birth of Mr.
Leon S. Ligoy.

4. We are not related by consanguinity or affinity to Leon S. Ligoy.

5. We are executing this Joint Affidavit to attest to the truth of the foregoing facts
of birth and to support the application for delayed registration of the same with
the Office of the Civil Registrar of Baras, Rizal and for whatever legal purpose
this may serve.
