(FOR DELAYED REGISTRATION OF BIRTH)

WE, RICO S. LORE and BONA B. MONTERON , both of legal age, Filipinos
and all residents of Dawis Norte, Carmen , Cebu, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and state that:

1. That we personally know MARK JAY D. AWING who was born


on April 27, 1992 at Lower Natimao-An, Carmen, Cebu to
spouses Venerando M. Awing and Emma D. Awing;

2. That due to negligence or inadvertence on the part of her parents


and the prevailing conditions then, her birth was not reported and
registered with the office of the Municipal Civil Registrar of Carmen,
Cebu immediately thereafter;

3. That we are fully aware and cognizant of the above-stated facts


because we are close friends and neighbors for a long period of time;
and,

4. That we are executing this Joint Affidavit to the best of our own
knowledge and to attest to the truth of all the foregoing facts, in
connection with the application for the late/ delayed registration of
the birth of the said MARK JAY D. AWING with the Office of the
Municipal Registrar of Carmen, Cebu and for whatever legal
purposes.

Cebu City, Cebu, Philippines, after having been duly sworn to in accordance with law, do hereby depose and state
that:

1. That we personally know ANTONIO MARTIN ICHON who was born on January
30, 1953 at Malitbog, Leyte to spouses Rosario Martin and Bienvenido Ichon.

2. That due to negligence or inadvertence on the part of his parents and the prevailing
conditions then, his birth was not reported and registered with the office of the Municipal
Civil Registrar of Malitbog, Leyte immediately thereafter.

3. That we are fully aware and cognizant of the above-stated facts because we are
close friends and neighbors for a long period of time.

4. That we are executing this Joint Affidavit to the best of our own knowledge and to
attest to the truth of all the foregoing facts, in connection with the application for the late/
delayed registration of the birth of the said Antonio Martin Ichon with the Office of the
Municipal Registrar of Dalaguete, Cebu and for whatever legal purposes.
