This 5S checklist can help you to evaluate how well you are doing in your 5S

process.

5S Rating:
Level 5 - Excellent condition;
Level 4 - Very Good, needs a little more work to attain excellent condition;
Level 3 - Ordinary workshop condition;
Level 2 - Below ordinary workshop condition. Needs positive efforts to correct situation;
Level 1 - Needs to change situation completely.
