NCP 1activity Intolerance N C P BY BHERU LAL

Rab Dino S/O Mola Bux, a 50-year-old farmer, was admitted to the ward with a chief complaint of abdominal pain gradually worsening over the past month. His medical diagnosis was vesicle calculus. The nursing assessment found he had activity intolerance related to his bladder stone as evidenced by pain. The short-term nursing goals were for the patient to gain strategies to manage his pain by the end of the nurse's duty. The long-term goal was for the patient to continue gaining strategies to manage his pain beyond the current nursing duty. Interventions included reassurance, IV maintenance, diet and hydration advice, hygiene and comfort measures, and ensuring proper medication was given.
