I, A B C, of legal age, married, Filipino, and a resident of

law, hereby depose and state that:

the Business Name Registration of the

Unfortunately, the said DTI Certificate erroneously interchanged

which is A B C, and which correct name appeared in my previous DTI
Certificates, as well as my government issued IDs such as my Drivers
License and Passport;

The name A C B, and the name A B C refer to one and the same
person, herein Affiant;
5.
I am executing this Affidavit to attest to the truthfulness of the
foregoing statements, to explain the error in my DTI Certificate of
Registration, and for whatever legal purpose this may serve.
