AFFIDAVIT OF ACKNOWLEDGMENT
OF DEPENDENTS
I, PO1 VENNIS O. OTCHIA, Filipino, of legal age, married, and a resident of
Poblacion Bonifacio, Misamis Occidental, after having been duly sworn to in accordance with
law, hereby depose and say, that:
1.

I am a member of the Philippine National Police (PNP) currently assigned at the

Provincial Public Safety Company (PPSC), Josefina, Zamboanga del Sur;


2.

I am married to SARVINA L. OTCHIA, having entered into marriage on June 15, 2013

and my son CLARENCE L. OTCHIA was born on October 13, 2014;


3.

I acknowledge them to be my true, lawful and only dependents.

4.

I am executing this affidavit to attest to the truth of the foregoing facts and for whatever

legal purpose it may serve.
