AFFIDAVIT OF LOSS
I, ELDEE R. BALOLONG, Filipino, of legal age, single and a resident of 203
C. Padilla St., Cebu City, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say; THAT:
That I am the holder of BDO CASH CARD RECEIPT issued to me by BDO
INSULAR AYALA CEBU BRANCH;
That the said BDO CASH CARD RECEIPT was lost due to misplaced from
my possession without my knowledge;
That despite diligent efforts exerted in order to save the said BDO
CASH CARD RECEIPT the same could no longer be recovered;
That I am executing this affidavit in order to attest to the truth of the
foregoing statements and for whatever legal purpose this may serve;
FURTHER AFFIANT SAYETH NONE.
IN WITNESS WHEREOF, I have hereunto set my hand this 28 th day of April
2016, at Cebu City, Philippines.
