CVA Activity Intolerance

1. The patient experienced a cerebrovascular accident (CVA or stroke) resulting in right hemiplegia and generalized weakness. 2. The nursing diagnosis is activity intolerance related to generalized weakness secondary to the CVA. Motor deficits caused by destruction of neurons in the brain are the most obvious effect of a stroke. 3. The plan is for the patient to participate willingly in necessary activities after 4 hours of nursing interventions, and to demonstrate a decrease in physiological signs of intolerance like heart rate and blood pressure remaining in the normal range after 2 days of interventions.
