Researchers are currently trying to understand why some men with prostate cancer go on to develop aggressive disease whilst others maintain slow growing tumors. Although endogenous genetic anomalies within the tumor cell are important, the prevailing view is that the tissue microenvironment as a whole is the determinant factor. Many studies have focussed on the role of soluble factors in modulating the nature of the tumor microenvironment. There is however a growing interest in the role of extracellular vesicles, including exosomes, as regulators of disease progression. A variety of resident cells, as well as infiltrating cells, all contribute to a heterogeneous population of exosomes within the tumor microenvironment. Studies focussing on the role of exosomes in prostate cancer are however relatively rare. In this review, evidence from various cancers, including prostate, is used to present numerous potential roles of exosomes in prostate cancer. Whilst further validation of some functions may remain necessary it is clear that exosomes play a major role in intercellular communication between various cell types within the tumor microenvironment and are necessary for driving disease progression.

The stroma plays a fundamental role in the function of different glandular systems. In the prostate, the stroma is responsible for the development and maintenance of the differentiated state of the epithelium. Nicotine induces tobacco dependence and promotes morphological alterations in the epithelial compartment. However, its effects on the prostate stroma are unclear. The objective of this study was to evaluate the morphology of the stromal microenvironment in the ventral prostate lobe of rats submitted to chronic nicotine administration. Twenty rats (Rattus norvegicus) were divided into two groups: 10 animals received subcutaneous nicotine and 10 animals received physiological saline by the same route. After treatment, samples were collected from the ventral prostate lobe, processed and submitted to histology, histochemistry, and ultrastructural analysis by transmission and scanning electron microscopy. The level of circulating testosterone was also analyzed. The results showed a significant increase in the density of type I collagen (56.3% to 85.9%, P < 0.01) and a decrease in the density of type III collagen (43.7% to 14.1%, P < 0.01). In addition, there was a qualitative increase in elastic fibers and in the number of smooth muscle cells with a secretory phenotype. Circulating testosterone levels were significantly reduced (898.3 to 363.1 ng/mL, P < 0.01). The results showed that nicotine modifies different components of the prostate stroma, suggesting that this drug is a risk factor for morphofunctional alterations in the prostate gland.

Activating mutations of the KIT juxtamembrane region are the most common genetic events in gastrointestinal stromal tumors (GISTs) and have been noted as independent prognostic factors. The impact of KIT mutation in other regions, such as the extracellular or kinase domains, is not well-defined and fewer than 30 cases have been published to date. One hundred twenty GISTs, confirmed by KIT immunoreactivity, were evaluated for the presence of KIT exon 9, 11, 13, and 17 mutations. The relation between the presence/type of KIT mutation and clinicopathological factors was analyzed using Fisher's exact test and log-rank test. Forty-four % of the tumors were located in the stomach, 47% in the small bowel, 6% in the rectum, and 3% in the retroperitoneum. To evaluate the effect of removal and/or exchange of intrastromal corneal ring segments (ICRS, Intacs); specifically the risk factors, degree of reversibility, and visual outcomes. Gimbel Eye Centres, Calgary, Edmonton, and Vancouver, Canada. Prospective cohort data were collected on 71 eyes that had ICRS placement by 3 surgeons at the Gimbel Eye Centres between August 1998 and July 1999. Data included patient symptoms, best spectacle-corrected visual acuity, uncorrected visual acuity (UCVA), manifest refraction, reason for removal or replacement, keratometry, corneal topography, and complications. Twenty-four eyes (33.8%) had exchange or removal procedures because of underresponse, 20 eyes; overresponse, 2 eyes; and induced astigmatism and shallow placement, 1 eye each. Secondary procedures were removal and replacement, removal with subsequent laser in situ keratomileusis, and segment repositioning. Two risk factors for secondary surgery were identified: high preoperative refractive astigmatism and postoperative spherical equivalent greater than +/-0.5 diopter (D). The 6 eyes that had ICRS removal stabilized to within +/-0.25 D of the original refractive error with no surgically induced astigmatism within 1 to 7 weeks (mean 4 weeks) and had a final UCVA of 20/20 or better. In the 15 eyes that had ICRS exchange procedures, the UCVA improved by at least 1 line and median UCVA improved from 20/40 to 20/20. Preliminary data from this study indicate that removal of segments resulted in reversal of the refractive effect.
