This paper presents a case report on the remarkable clinical and biological effects of “ASEA redox supplement” (ASEA-rs) oral solution (co-administered with L-carnitine and omega-3 fatty acids plus multivitamins dietary supplements) in a ~3-year-old boy with Duchenne muscular dystrophy (DMD) from Romania. ASEA-rs was tested on both animals and humans. In vitro studies on various human cells showed that ASEA-rs is a very potent NRF2 selective activator (with transient effect): the studies conducted in vivo (on both animals and humans) also support this main pharmacological mechanism of ASEA-rs, with no toxicity up to high doses (in contrast with corticosteroids which are usually reserved in Romania for DMD children with age above 4 years, given their toxicity profile and adverse effects including immunosuppression, growth delay, osteopenia, osteoporosis and overweight) and a satisfactory bioavailability especially in the vital organs (in which NRF2 also reaches its highest intracellular cytoplasmatic concentrations), which assures the strong NRF2 activation effects of ASEA-rs indirectly observed in vivo. ASEA was clearly demonstrated to activate tissular lipases (probably also via NRF2 pathway) and to significantly increase some fatty acids serum levels that are further internalized by skeletal muscles and myocardium and used as “fuel” by muscular cells (myocytes) (and cardiomyocytes), partially sparing the glycogen reserves of myocytes/cardiomyocytes and so raising the resistance of skeletal muscles to effort and possibly the resistance and contractility of myocardium. Based on its demonstrated potent selective NRF2 activation effect, its beneficial effects on muscle effort resistance and its safety profile, I have prescribed ASEA-rs to this ~3-year-old child with DMD, with a minimal set of clinical signs at his age, but with significant biological alternations in the biochemical markers of muscular damage (rhabdomyolysis): the results (after the first 3 months with ASEA-rs, associated with L-carnitine and omega-3 fatty acids plus multivitamins supplements) were a significant reduction in the creatine (phospho)kinase (CK/CPK) and CK-MB isoform serum levels. Bedwetting is a common disorder in children, which cause embarrassment and concern for the child and parents together. Parents' concepts about the causes and treatment vary, as well as their reactions towards their children complaining from this disorder. The study involved six basic schools in Karari locality, and included a number of parents of 386 children. The study found that two-thirds of parents participating in the study believe that the anxiety and drinking excessive fluids are the most important causes, while 36% of parents believed that the lack of female genital mutilation is important cause of bedwetting in females. 30% of the parents believe that it is due to organic disorder. The study also showed that 20% of parents were suffering from the same problem when they were children. As for the parents about the concepts of treatment, 72% believe that the problem will disappear with time, while 60% believed in the importance of the medical treatment; however, the actual percentage of those who visited a doctor in order to solve the problem was 32%. And back to the trends of the parents toward this problem: 29% of the parents were abusing their children verbally at the time of bedwetting while 60% feel ashamed and discomfort. The study also found a statistically significant relationship between parents concepts and the relationship parenting of the child in favor of mothers, and also if the child is suffering from bedwetting actually , while there was no statistically significant relationship between parents concepts and the age of the parents, their level of education or between perceptions and Sex of the child .As observed in the study the high proportion of bedwetting among children in the study sample that reach 42%, and this requires extensive study involving a larger sample of children to highlight the real ratio and comparing it with the global proportions

مستخلص الدراسة :
      يعتبر التبول اللاإرادي من الإضطرابات الشائعة لدى الأطفال , والتي تسبب في الحرج والقلق للطفل والوالدين معاٌ . تختلف مفاهيم الآباء حول الأسباب وطرق العلاج , كما تختلف ردات أفعالهم تجاه أطفالهم المصابين بهذا الاضطراب . شملت الدراسة  بعض مدارس الأساس بمحلية كرري , وشملت  الآباء لعدد 386 طفل في 6 مدارس أساس . وخلصت الدراسة إلى  أن ثلثي الآباء المشاركين في الدراسة يعتقدون أن القلق وشرب السوائل المفرطة هي من أهم المسببات بينما يعتقد 36% من الآباء أن عدم ختان الإناث من أسباب التبول اللاإرادي في الإناث وقلت النسبة إلى 30% للإعتقاد بوجود سبب صحي . كما بينت الدراسة أن 20% من الآباء كانوا يعانون من  نفس المشكة عند الصغر.
        أما عن مفاهيم الآباء حول العلاج فإن 72%يعتقدون أن المشكلة ستزول مع الزمن بينما يعتقد 60% في وجود علاج طبي إلا أن النسبة الحقيقية للذين زاروا الطبيب من أجل حل المشكلة كانت 32%. و رجوعاٌ الى ردات أفعال الآباء فإن 29% من الآباء كانوا يعنفون أطفالهم لفظياٌ عند حدوث التبول اللاإرادي  بينما كان .60% يشعرون بالخجل وعدم الإرتياح . وقد خلصت الدراسة أيضاٌ إلى وجود علاقة دالة إحصائياٌ بين مفاهيم الآباء والعلاقة الوالدية للطفل لصالح الأمهات , وأيضاٌ إذا كان الطفل يعاني من التبول اللاإرادي  أفعالياٌ ومفاهيم والديه, بينما لا توجد أي علاقة دالة إحصائياٌ بين مفاهيم الآباء وعمر الوالدين ,ومستواهم التعليمي أو بين مفاهيمهم و جنس الطفل.ومن الملاحظ في نتائج الدراسة إرتفاع نسبة التبول اللاإرادي بين أطفال عينة الدراسة لتصل إلى 42% وهذا يتطلب دراسة واسعة تشمل عينة أكبر من الأطفال لإبراز النسبة الحقيقية  ومقارنتها بالنسب العالمية.

The rapid spread of the covid19 pandemic has posed a dilemma for us, should we vaccinate children against SARSCoV2 infection, with which type of vaccine, or should children be exposed to natural infection and stay unvaccinated? The prevention of COVID19 is more effective than the treatment of a child with COVID19 or long-COVID19, which is a kind of recommendation for vaccination of children against SARSCoV2. A new circumstance is the poor efficacy of previous vaccines, in adults, against new strains of SARSCoV2 and the fact that, in the September wave of the COVID19 pandemic, a worrying number of children became ill compared to previous waves. So far, only a few countries in the world have achieved vaccination coverage of over 80%.

REZUMAT/ABSTRACT Acest proiect propune perfectionarea si testarea softului propriu numit formal "Pediatrul electronic" ("Electronic pediatrician"/"EPed"), un software de diagnostic si tratament pediatrice asistate de calculator. EPed se afla in acest moment la stadiul de varianta preliminara (demo). Imi propun sa perfectionez acest soft printr-un program doctoral autofinantat, atat prin update-ul consistent al bazei de date medicale (cu care lucreaza EPed) cat si prin rafinarea algoritmilor de diagnostic si tratament automate: dupa aceasta perfectionare, EPed va fi testat pe sectia de "Boli infectioase pediatrice non-COVID-19" (BIC-NC) a Spitalului Judetean de Urgenta Targoviste (SJUT), unde autorul isi desfasoara in prezent activitatea clinica, cu acordul managementului SJUT (dar si al serviciilor IT si GPDR). Testarea EPed va consta in general in a prezice (in paralel cu medicul curant) diagnostice si tratamente plauzibile pentru orice pacient pediatric (spitalizat pe sectia BIC-NC), care diagnostice si tratamente propuse de calculator (prin EPed) vor fi apoi confruntate/comparate cu cele propuse si aplicate de medicul curant: scopul cercetarii EPed este acela de a testa fiabilitatea unui astfel de soft in a ajuta constant medicii curanti ai sectiei BIC-NC (dar potential si al oricarei sectii pediatrice/UPU pediatric/BIC-NC din tara) in procesul deciziilor clinice (de diagnostic si tratament), pentru a creste (la un maxim posibil) precizia, randamentul si viteza acestor decizii clinice. EPed ar fi primul software electronic (sistem computerizat) de acest tip (cu specific pediatric) din tara si, dupa testarea in SJUT (in BIC-NC), va fi propus pentru a fi extins si in alte spitale din tara (pe sectiile de pediatrie) si chiar propus spre a fi folosit in sistemul de invatamant medical mediu si superior din Romania, ca instrument pedagogic cu o mare utilitate potentiala in antrenarea studentilor si rezidentilor viitori medici.

Purpose: 1p36 deletion syndrome is the most common terminal deletion syndrome, with an incidence of 1/5,000 newborns. But 22q13 duplications seem to be exceedingly rare. A combined 1p36 deletion and 22q13 duplication was more rarely observed and presented variability of clinical features, which increases the importance of reporting additional cases in order to better characterize genotype-phenotype correlations.

Methods: A boy and a fetus from a single family with combined 1p36 deletion and 22q13 duplication characterized by array CGH and MLPA were described here.

Results: The proband presented severe developmental delay, hypotonia, epilepsy, feeding difficulties with failure to thrive, tracheal malformation,
clinodactyly, strephexopodia, fair skin, facial dysmorphism and died at 1 year of age. Array CGH uncovered a 9.3-Mb deletion of 1p36 plus a 6.655-Mb duplication of 22q13in the proband. This rearrangement was confirmed by MLPA. When his mother was pregnant again, array CGH detected an almost identical rearrangement with that of the proband. She terminated the pregnancy at 24 weeks gestation. The fetus was female and had similar facial dysmorphism as the proband. Karyotypes of parents are all normal.

Conclusions: Most of the features in the proband were similar to those associated with both isolated 1p36 deletions and 22q13 duplications.
However, tracheal malformation, fair skin and strephexopodia were only observed in our patient. Given that 22q13 duplications are rare and not as well characterized as 1p36 deletions, we attributed these features to 22q13 duplications. Importantly, we emphasize importance of prenatal diagnosis of females who had such abnormal pregnancy.

Conclusion: This evaluation of a state interprofessional OHE program shows evidence of program diffusion and identifies facilitating factors and barriers to having medical professionals provide OHPS.

Background: Proper feeding of preschool children can promote their optimal growth and development. As preschoolers spend long time in daycare centers, the understanding of their feeding practices becomes more important. The aim of this study is to assess the teachers' observations and perspectives in preschoolers' feeding behaviors and practices. Methods: This cross sectional study was conducted in 71 daycare centers in Khartoum locality among preschool teachers in August 2016. Results: demographic characteristics of the teachers showed that 43.7% of them had less than 5 years' experience in their work, with two third had not trained in child nutrition before. About their observation in the feeding practices: 94.4% of them observed daily repeated types of food, 38% recognized loss of appetite as problem, 7% observed poor quality of food, and 15.5% of teachers noticed that children like to eat the other types of food brought by their colleagues. 47.8% of daycare centers offer snacks, while 62% of the teachers practice food sharing between children. The quality of food provided was composed mainly of cereals and proteins with complete absence of vegetables and fruits.79% of the teachers justify the main cause of loss of appetite was eating or drinking sugary foods, while 24% attributed that to physiological causes, 20% behavioral causes, 14% organic illness, and 2.8% they did not know the real cause. Regarding the teachers' response to these observations; 86% of teachers inform the parents or home caregivers, 33.8% had behavioral approaches to manage the feeding problems, while 7% of teachers consulted physician. Conclusions: Preschool teachers have a crucial role in young children feeding practices and behaviors, as they can influence their feeding behaviors through their actions and words, these besides conducting their observations to the home. Training teachers and making programs about the proper ways of healthy eating habits could be implemented and encouraged.

Background: The gold standard to assess hyperbilirubinemia in neonates remains the serum bilirubin measurement. Unfortunately, this is invasive, painful, and costly. Trying to overcome these drawbacks, non-invasive methods of bilirubin measurements have been proposed. Our aim  was  to  assess  the  agreement  between  capillary  sample  gas  analyzer  results,  total  serum  bilirrubin  levels  and  transcutaneous  bilirrubin  results.Methods: the transcutaneous bilirubin (TCB) measurements were performed on the infant’s sternum and serum and capillary bilirubin were determined simultaneously. The agreement between both methods was assessed by Spearman ́srank correlation coefficient.Results:  A  total  of  217  measurements  were  performed  in  75infants.  Median  bilirrubin  measurements  were  6,127  mg/dl  (for  serum  samples) 6,684 mg/dl (gasometry) and 4,371 mg/dl (TCB). A significant correlation was observed between serum samples and analyzed by  gasometry  (Spearman ́s  Rho  0,895;  95%  IC  CCI:  0.955).  Correlation  was  strong  but  lower  between  serum  samples  and  TCB  (Spearman ́s  Rho 0,881; 95% CI CCI: 0.847) and gasometry and transcutaneous samples (Spearman ́s Rho 0,914; 95% CI CCI: 0,834). Analyzing for ranges; correlation is higher in 5-9 mg/dl interval comparing serum samples and analyzed by gasometry (95% CI CCI: 0.882). Conclusion:  transcutaneous  bilirubin  and  determined  by  gasometry  could  be  alternatives  to  total  serum  for  early  diagnosis,  proper  management of the neonatal jaundice, increase quality of care, improve in parent satisfaction, and an overall decrease in hospital charges.

This paper proposes an important RED list of what to NOT do in pediatrics, with the conviction that this red list should be taught from the first year of medicine and repeated in every single year of medical teaching, in all medical universities from all countries worldwide: the work-hypothesis (and the main motivation of this paper) is that insisting on this red list in all medical teaching systems may significantly decrease the rate of medical/pediatric malpractice thus may significantly improve the health status of any child population from any country. This paper will be periodically updated so that to increase the efficiency of this proposed medical teaching method based on the principle that any medical specialty should be taught starting (and repetitively insisting!) on what to NOT do in that medical specialty practice! This paper continues the line of other medical articles/preprints of the same author. This list is mainly addressed to young medical students, but also to young medical doctors, nurses etc. from the beginning of their careers.

Introduction: Youth suicide is one of the top three causes of death among age group between 15 and 24 years old. The
known risk factors include depression, hopelessness, dysfunctional families, substance abuse, school failure and harassment.
However, research reports in relation to the protective factors to prevent adolescent from suicidal attempts are still lacking.
The purpose of this study is to detect (if any) the protective effect of physical activity and the habit of eating breakfast on
children suicidal attempts.
Methods: This study adopted a secondary data analysis approach, in which data was extracted from the 2013 Youth Risk
Behaviour Surveillance System to detect the association effect between children's physical activity level and the habit of
eating breakfast with suicidal attempts.
Results: The increase number of days of physical activity and the habit of eating breakfast were both significantly related
to the declining trend in suicidal attempts (p<0.0001). Only the effect of habit of eating breakfast on suicidal behaviour
remained a protective factor for suicidal attempts after the adjustment (p<0.001).
Conclusions: Our results illustrate that the physical activity has a marginal effect on suicidal attempts among males only.
After adjusting other risk factors, the habit of eating breakfast shows a strong effect on preventing children from having
suicidal attempt and also suggests a dose effect for the days of eating breakfast. For a child who skips breakfast for a week
would have 1.7 higher risk of suicidal attempt compared with another child who eats breakfast every day in a week.

Background &Objectives: Antiemetic drugs can be used in complicated cases of vomiting secondary to gastroenteritis. There is no previous study dealing with the use of pyridoxine in treatment of vomiting in pediatrics. This Studyintendsto review the effects of intravenous pyridoxine on control of gastroenteritis related vomiting. Methods: This study is a single blind randomized clinical trial with placebo. A total number of 147 children diagnosed with gastroenteritis induced vomiting and admitted in Amirkabir hospital from August to December 2011 were studied and divided to two case andcontrol groups. In the control group intravenous fluid with distilled water (placebo) was used and the pyridoxine was given along with intravenous fluid therapy in case group.
