The use of, and interest in, total knee replacements (TKR) has been growing over the last few
decades. Loosening and migration of tibial components have been identified as one of the
primary causes of failure in the proximal tibia. Clinical studies show the use of metal
implants as one of the primary methods for the treatment of knee joints and associated bone
defects. Alignment and fixation techniques play an important role in achieving high success
rates. Defective bone stock requires the use of augments to stabilise the tibial plate. In these
cases, current clinical practice is to use an extended implant stem to ensure stability. The
problem with this is that it reduces the potential for future knee revision
In this research Finite Element Analysis (FEA) has been used to undertake virtual in-vivo
assessment of various configurations of augmented and non-augmented TKR that can be used
for the treatment of tibial defects. These configurations are based on a standard tibial insert,
namely a fixed bearing revision tibial tray. This has provided insight and information that can
be used to improve surgical decision making when dealing with defective bone stock.
The 3D FE models of a non-defect TKR with a fixed bearing tibial insert showed a stable
construct with stresses lying within an allowable threshold. The use of a stem extension
generally showed a reduction in stress levels in the cancellous bone contributing to an
increase in stress shielding and thus it is recommended that these are not used unless there is
some other overriding clinical requirement.
Further, the analysis demonstrated that, contrary to some clinical opinion, wedge
augmentation (rather than block augmentation) may provide a better approach to treat the
defect. This was largely due to improved cement stress distribution caused by a mechanism
termed “reverse-shear”.
The use of a cement augment was found to provide a more favourable stress distribution in
the cancellous bone. However, metal augments have been recommended as the cement
augment was shown to operate too close to its fatigue endurance limits.

Introduction. The differences in the supporting structure of the implant make them more susceptible to inflammation and bone loss when plaque accumulates as compared to the teeth. Therefore, a comprehensive maintenance protocol should be followed to ensure the longevity of the implant. Material and Method. A research to provide scientific evidence supporting the feasibility of various implant care methods was carried out using various online resources to retrieve relevant studies published since 1985. Results. The electronic search yielded 708 titles, out of which a total of 42 articles were considered appropriate and finally included for the preparation of this review article. Discussion. A typical maintenance visit for patients with dental implants should last 1 hour and should be scheduled every 3 months to evaluate any changes in their oral and general history. It is essential to have a proper instrument selection to prevent damage to the implant surface and trauma to the peri-implant tissues. Conclusion. As the number of patients opting for dental implants is increasing, it becomes increasingly essential to know the differences between natural teeth and implant care and accept the challenges of maintaining these restorations.

There has been a tremendous increase in the use of dental implants to replace missing teeth, in the last decade. This has led to a dramatic rise in the number of implant complications primarily misguided and incorrectly placed implants. This complication could be easily overcome by using surgical guides for implant positioning. Although conventionally made surgical guide are being used, the clinical outcome is often unpredictable, and even if the implants are well placed, the location and deviation of the implants may not meet the optimal prosthodontic requirements. High accuracy in planning and execution of surgical procedures is important in securing a high success rate without causing iatrogenic damage. This can be achieved by computed tomography, 3D implant planning software, image‑guided template production techniques, and computer‑aided surgery. This article evaluates about the various systems of conventionally made surgical guides using radiographs and also newer computer generated surgical guides in detail.

Background The incidence of implant-related inferior alveolar nerve injuries (IANI) is steadily increasing within the UK population.Aims This study prospectively reviewed thirty cases (35% male; 65% female) of implant-related IANI seen in a specialist nerve injury clinic.Methods Neurosensory examinations were carried out to ascertain a quantifiable rating of the perception, pain profiling and functional difficulties. Data were analysed using SPSS software.Results Patients were aware of signing consent forms for the surgery in 11 cases and 8 of those felt they were not explicitly warned about nerve injury. Over 70% of patients were referred after six months post injury. Implant surgery planning involved intra-oral films only (30%), CBCT (10%), dental pantomograph (50%) and long cone peri-apical radiographs (48%). The alternatives in the management of infantile glaucoma refractory to conventional treatment are quite limited and unrewarding. We did a retrospective study of 49 patients (53 eyes) under 12 years of age with advanced, uncontrolled glaucoma who each underwent implantation of a single-plate Molteno implant in one stage. Over the follow-up period ranging from 6 months to 3 years (mean 18 months), 36 of the 53 eyes (68%) had an intraocular pressure less than or equal to 21 mm Hg and were considered a success. Fourteen eyes (26%) required further glaucoma surgery, 2 eyes developed severe chronic hypotony, and 1 eye lost light perception following endophthalmitis. The number of anti-glaucoma medications was decreased from a mean of 2 (+/- 1) to 1 (+/- 1). The probability for success of the Molteno implant in controlling glaucoma increased with the age of the patient. Six eyes (11%) were late failures, most probably due to fibrous encapsulation of the bleb. Complications observed were: prolonged hypotony and flat anterior chamber, retinal detachment, migration of the tube, erosion of the tube through the conjunctiva, and an ingrowth of fibrous tissue in the anterior chamber. We consider the Molteno implant to be a reasonable option in the management of difficult cases of infantile glaucoma.

The aim of this study was to evaluate the effects of cortical bone thickness and implant length on strain in the surrounding bone and micromotion at the bone-implant interface in single immediately loaded implants. Experiments with rapid prototyping technique and finite element simulations were performed to evaluate bone strain and micromotion at the bone-implant interface. Implants (13-mm long) were embedded into models with three thicknesses of cortical bone (0.5, 1.7, and 3.4 mm), and implants with different lengths (11.5 and 15.0 mm) were inserted into models with 1.7 mm of thickness of cortical bone. Background: In developing countries, there is an increase in awareness among the population regarding dental implant treatment. However, there is paucity in studies on the relationship between patients' biographic information, and different aspects of dental implant treatment. Aims: This study's purpose is to determine the influence of Iraqi patients' age and gender on various clinical aspects in dental implant treatment. The recorded data included: patient age, gender, implant zone, implant timing, implant side, dental implant system, sinus lift, bone augmentation, implant length, and implant diameter. SPSS Ver. 25 was used to perform the statistical analysis. Methods and Materials: Biographic data of 196 Iraqi patients who attended a private dental implant center in Baghdad from 7.1.2016 to 30.4.2020 was recorded. During this period 348 dental implant procedures were completed. Results: There was a highly significant negative relationship (p=0.006) between implant length and patient's age, and a significant positive relationship (p=0.028) between implant size and patient's age. A statistically significant relationship (p=0.013) was found between the patient's age and dental implant zone and dental implant timing (p=0.016). A significant difference was found between the two genders in terms of dental implant timing (p=0.017). Conclusion: females are better candidates for dental implant treatment. Older patients seem to benefit more from shorter, and, subsequently, wider dental implants. Also, older patients tend to have lower anterior dental implants.

The commonly used implantation technique involves a 2-stage surgical protocol for load-free and submerged healing to ensure predictable osseointegration. This article presents a new approach that benefits from previous bone and soft tissue protocols. It combines new surgical and insertion techniques to perform immediate loading with delayed implant placement, avoiding the problems of discomfort, inconvenience, and anxiety associated with healing periods. It maintains a similar success rate to the present common technique. After osteotomy, bone healing passes through 3 stages (i.e., inflammation, fibrous tissue, and maturation). In the new approach, implant insertion is performed 2 weeks after drilling (i.e., during the fibrous phase). The insertion timing is so selected because collagen formation and revascularization represent an acceptable implant bed configuration. Flap surgery offers the optimal osteotomy approach, but esthetic outcomes are definitely compromised. Accordingly, adopting the punch technique alleviates several postoperative problems, such as pain, tenderness, and swelling. Moreover, the punch technique avoids the sutures needed to adjust the gingiva around the abutment. It provides both cosmetic and functional properties.

This study compared the speed and strength of osseointegration and osteoconductivity between an oxidized experimental magnesium (Mg) implant, an oxidized commercially available TiUnite implant, and a dual acid-etched surface Osseotite implant. The aim was to investigate which surface properties enhance bone response to implants, and thereby to test a biochemical bonding theory. A total of 60 screw implants (20 of each design) were inserted through 1 cortex into the tibiae of 10 rabbits. Surface chemistry, oxide thickness, morphology, crystal structure, and surface roughness were evaluated. After healing times of 3 and 6 weeks, all bone implants were unscrewed with removal torque (RTQ) devices, and the bone specimens were subjected to histomorphometry. A missing or deficient buccal alveolar bone plate is often an important limiting factor for immediate implant placement. Titanium dental implants enhanced with porous tantalum-based trabecular metal material (PTTM) are designed for osseoincorporation, a combination of vascularized bone ingrowth and osseointegration (bone on-growth). The aim of this study was to develop a protocol to analyze the microstructure of mandibular and maxillary bone in association with implant placement in ectodermal dysplasia (ED) and anodontia conditions compared to patients not suffering from such conditions. This study was not additionally invasive, since the bone harvesting was completed at the time and site of implant placement. : The objective of this study was to evaluate and compare 3 impression techniques for osseointegrated implant transfer procedures. : (1) Group Splinted with Acrylic Resin (SAR), impression with square copings splinted with prefabricated autopolymerizing acrylic resin bar; (2) Group Splinted with Light-Curing Resin (SLR), impression with square copings splinted with prefabricated light-curing composite resin bar; (3). Group Independent Air-abraded (IAA), impression with independent square coping aluminum oxide air-abraded. Impression procedures were performed with polyether material, and the data obtained was compared with a control group. These were characterized by metal matrix (MM) measurement values of the implants inclination positions at 90 and 65 degrees in relation to the matrix surface. Readings of analogs and implant inclinations were assessed randomly through graphic computation AutoCAD software. Experimental groups angular deviation with MM were submitted to analysis of variance and means were compared through Tukey's test (P < 0.05). : There was no statistical significant difference between SAR and SLR experimental groups and MM for vertical and angulated implants. Group IAA presented a statistically significant difference for angulated implants. : It was concluded within the limitations of this study, that SAR and SLR produced more accurate casts than IAA technique, which presented inferior results.
