Giriş ve Amaç: Siberkondri, internette tekrarlayan tıbbi bilgi arama sonucu sağlık anksiyetesinin şiddetlenmesidir. Siberkondri ekonomik maliyeti arttırmasının yanı sıra sağlık hizmetlerinin fazla kullanılmasına da neden olmaktadır. Ülkemizde de yaygın olduğu düşünülen siberkondri ile ilgili yapılan çalışmalar sınırlıdır ve bu sorunun araştırılması için uygun ölçüm araçları gerekmektedir. Bu çalışmanın amacı, Siberkondri Ciddiyet Ölçeği (SCÖ)’nin ve Kısa Formu (SCÖ-15)’nun üniversite öğrencileri üzerinde geçerlilik ve güvenilirliğinin değerlendirilmesidir.

Gereç ve Yöntem: Metodolojik tipteki bu araştırma Pamukkale Üniversitesi merkez yerleşkedeki (sağlıkla ilgili fakülteler/yüksekokullar hariç) öğrenciler üzerinde yapılmıştır. Örnekleme alınacak kişi sayısı, ölçekteki madde sayısının en az 5-10 katı kadar kişi sayısına ulaşılması gerekliliğine ve geçerlilik-güvenirlilik çalışmalarında örneklem büyüklüğünün 500’ün üzerinin çok iyi kabul edilmesine dayanarak belirlenmiştir. Araştırmaya 4 fakültenin ve 1 meslek yüksekokulunun son sınıf öğrencileri dâhil edilmiştir. Toplamda 1147 kişiye ulaşılmış, ancak ölçek sorularının %10’undan azına cevap vermiş olan 38 kişi çalışma dışında tutularak 1109 kişi analize dahil edilmiştir. 556 kişiye SCÖ, 553 kişiye ise SCÖ-15 uygulanmıştır. Veriler, katılımcıların sözel onamları alındıktan sonra 10-14 Ekim 2016 tarihleri arasında toplanmıştır. Veri toplama işlemi sosyodemografik özellikler ve sağlık hizmet kullanımını sorgulayan 24 soruluk anket formu ve ölçekler ile yapılmıştır. Siberkondri Ciddiyet Ölçeği (SCÖ) kişilerin siberkondri durumlarının değerlendirilmesi için McElroy ve Shevlin tarafından 2014’te geliştirilmiştir. Ölçeğin Türkçe geçerlilik ve güvenilirliği yetişkinler üzerinde 2016’da Uzun ve Zencir tarafından yapılmıştır. Barke ve arkadaşları ise 2016’da ölçeğin kısa formunun (SCÖ-15) geçerli ve güvenilir olduğunu göstermiştir. SCÖ ve SCÖ-15 zorlantı, aşırı kaygı, aşırılık, içini rahatlatma ve doktora güvensizlik alt boyutlarını içeren beşli likert tipinde ve sürekli ölçeklerdir. SCÖ 33 önermeden SCÖ-15 ise 15 önermeden oluşmaktadır. Puan aralıkları SCÖ için 33-165 ve SCÖ-15 için 15-75’dir. SCÖ ve SCÖ-15 yapı geçerliliği için açımlayıcı ve doğrulayıcı faktör analizleri (AFA, DFA) kullanılmıştır. Güvenilirlik ise Cronbach alfa iç tutarlılık ile sınanmıştır. Veri analizi için SPSS ve LISREL programları kullanılmıştır. Çalışma öncesinde Pamukkale Üniversitesi Tıp Fakültesi Girişimsel Olmayan Klinik Çalışmalar Etik Kurul izni ve gerekli kurumsal izinler alınmıştır.

Bulgular: SCÖ için yapılan AFA sonucunda KMO katsayısı 0,90, Bartlett Testi sonucu 6621,668 ile anlamlı olarak yüksek bulunmuştur (p<0,001). AFA sonucuna göre ölçeğin faktör yük değerlerinin 0,311 ile 0,780 arasında değiştiği ve beş faktörden oluştuğu belirlenmiştir. Beş faktörde toplanan yapının açıkladığı toplam varyans miktarı %49,8’dir. Takiben ölçeğin orijinal alt boyutlarını içeren önermeler DFA ile yeniden sınanmıştır. Uyum indeksleri SRMR=0,061, GFI=0,85, RMSEA=0,066, CFI=0,94, NNFI=0,94 olarak saptanmıştır. Uyum indekslerinin mükemmel veya kabul edilebilir düzeyde olduğu ve 5 faktörlü yapıyı doğruladığı gösterilmiştir. SCÖ’nün güvenirliliği için hesaplanan Cronbach alfa katsayısı toplamda 0,87 iken alt boyutlarda 0,60 ile 0,85 arasında değişmektedir. SCÖ-15 için yapılan AFA sonucunda KMO katsayısı 0,78, Barlett Testi sonucu 2242,344 ile anlamlı bulunmuştur (p<0,001). AFA sonucuna göre ölçeğin faktör yük değerleri 0,310 ile 0,849 arasında değişmekte olup beş faktörden oluştuğu belirlenmiştir. Beş faktörde toplanan yapının açıkladığı toplam varyans miktarı %63,94’tür. Alt boyutta toplanan tüm önermeler orijinal ile tam uyum içerisindedir. Takiben yapılan DFA ile elde edilen uyum indekslerinin SRMR=0,070, GFI=0,93, RMSEA=0,070, CFI=0,94, NNFI=0,92 ile mükemmel veya kabul edilebilir düzeyde olduğu ve 5 faktörlü yapıyı doğruladığı saptanmıştır. Cronbach alfa katsayısı toplamda 0,62, alt boyutlarda 0,59 ile 0,75 arasında değişmekte olup SCÖ’ye göre daha düşüktür.

Sonuç ve Öneriler: SCÖ’nün ve SCÖ-15’in üniversite öğrencilerinin siberkondri düzeyini ölçmede geçerli ve güvenilir ölçme araçları olduğu gösterilmiştir. SCÖ-15’in geçerlilik ve güvenirliliğinin farklı gruplarda yeniden sınanmasına ihtiyaç vardır.

Giriş ve Amaç: İnternette tekrarlayan tıbbi bilgi arama sonucu sağlık anksiyetesinin şiddetlenmesi siberkondri olarak tanımlanmaktadır. Siberkondri sağlık hizmetlerinin fazla kullanılmasına ve ekonomik maliyet artışına sebep olmaktadır. Üniversite öğrencilerinin interneti daha sık kullanmalarının siberkondri açısından risk oluşturabileceği ve sağlık hizmet kullanımını etkileyeceği öngörülmektedir. Bu çalışmanın amacı, bir üniversite son sınıf öğrencilerinde siberkondri düzeyi ve etkileyen faktörlerin belirlenmesidir.

Gereç ve Yöntem: Kesitsel tipteki bu araştırmanın evrenini bir üniversite merkez yerleşkede (sağlıkla ilgili fakülteler/yüksekokullar hariç) öğrenim gören öğrenciler oluşturmaktadır. Literatüre göre siberkondri puan ortalaması 60-75 arasında değişmektedir. Bu çalışmada örneklem büyüklüğü; beklenen siberkondri puanı 72, standart sapma 15, güç %80 ve alfa hatası %5 kabul edilerek 348 olarak bulunmuştur. Küme örneklem yöntemi kullanılması nedeniyle desen etkisi 2 alınarak toplamda 696 kişiye ulaşılması hedeflenmiştir. Üniversite merkez yerleşkesinde bulunan 16 fakülte ve 6 yüksekokuldan sağlıkla ilgili 3 fakülte ve 2 yüksekokul hariç tutulmuş, kalan 13 fakülte ve 4 yüksekokuldan ise 4 fakülte ve 1 meslek yüksekokulu basit rastgele yöntemle belirlenmiştir. Belirlenen her bir okulun tüm bölümleri arasından 1 bölüm yine basit rastgele yöntemle seçildikten sonra seçilen bölümlerin son sınıf öğrencileri çalışmaya dahil edilmiştir. Veri toplamada sosyodemografik özellikler ve sağlık hizmet kullanımını sorgulayan anket formu (24 soru) ile Siberkondri Ciddiyet Ölçeği (SCÖ) kullanılmıştır. SCÖ Türkçe geçerlilik ve güvenilirliği 2016’da yapılan, 5’li likert tipinde, 33 önermeden oluşan sürekli bir ölçektir. Katılımcıların verdikleri yanıtlara göre her bir sorudan elde ettikleri puanlar toplanarak toplam siberkondri puanları hesaplanmıştır (en düşük 33 ve en yüksek 165 puan). SCÖ puanı yükseldikçe siberkondri düzeyi artmaktadır. Tanımlayıcı istatistikler için sayı, yüzde, ortalama ve standart sapmalar verilmiş, siberkondri düzeyini etkileyen faktörlerin belirlenmesinde Mann-Whitney U ve Kruskal-Wallis testleri ile lineer regresyon analizi (backward) yapılmıştır. p<0,05 istatistiksel olarak anlamlı kabul edilmiştir. Çalışma öncesinde girişimsel olmayan klinik araştırmalar etik kurul izni ve gerekli kurumsal izinler alınmıştır. Veri, katılımcıların sözlü onamları alındıktan sonra 10-14 Ekim 2016 tarihleri arasında toplanmıştır.

Bulgular: Araştırmaya 580 kişi (%83,3) katılmıştır. Katılımcıların yaş ortalaması 21,2±1,7 olup %54,5’i kadın ve %98,6’sı bekardır. Öğrencilerin %31,7’si yurtta yaşamaktadır. Öğrenim görülen okullar iktisadi ve idari bilimler fakültesi %20,7, eğitim fakültesi %20,3, teknik bilimler meslek yüksekokulu %20,3, mühendislik fakültesi %19,5 ve fen-edebiyat fakültesi %19,1’dir. Öğrencilerin %50,3’ünün annesi, %41,6’sının ise babası ilkokul mezunudur. Sigara içenler %34,4’tür. Kronik hastalığı olanlar %5,2, ailesinde kronik hastalık olanlar %27,2 ve ailesinde engelli/bakıma muhtaç kişi olanlar %4,9’dur. Günde internette geçirilen süre ortalaması 3,4±2,5 saattir. Basılı yayın takip edenler %30,5, görsel yayın takip edenler %38,0’dır. Öğrencilerin %12,8’i son 1 yıl içinde sağlıkla ilgili bir toplantıya katılmıştır. Ailesinde sağlıkçı bulunanlar %17,9’dur. Son bir yılda hekim önerisi dışında tetkik yaptıranlar %19,9, hekim önerisi dışında ilaç kullananlar %39,5, eczane dışında ilaç/tedavi amaçlı ürün kullananlar %16,1, alternatif tedavi kullananlar %3,1 ve son bir ayda muayene olanlar %43,7’dir. Öğrencilerin siberkondri puan ortalaması 79,4±16,9’dur. Cinsiyete göre siberkondri puanları arasında (Kadın:80,0±16,7, Erkek:78,7±17,0) istatistiksel olarak anlamlı fark saptanmamıştır (p=0,383). Okullara göre siberkondri puan ortalamaları fen-edebiyat fakültesi 82,6±17,6, eğitim fakültesi 80,8±15,8, iktisadi ve idari bilimler fakültesi 79,9±17,8, mühendislik fakültesi 79,4±14,2 ve teknik bilimler meslek yüksekokulu 74,2±17,7 olup aradaki fark istatistiksel olarak anlamlıdır (p=0,011). Siberkondri puanı ile sağlık hizmet kullanımı arasındaki ilişki analiz edildiğinde; son bir yılda hekim önerisi dışında tetkik yaptıran (p:0,013), hekim önerisi dışında ilaç kullanan (p<0,001), eczane dışı ilaç/tedavi amaçlı ürün kullanan (p:0,004), alternatif tedavi kullanan (p:0,007) ve son bir ayda muayene olan (p:0,003) öğrencilerin siberkondri puanları istatistiksel olarak anlamlı düzeyde yüksek bulunmuştur. Yaş, cinsiyet, okul, sigara kullanımı, sağlıkla ilgili basılı ve görsel yayın takip etme ile internet kullanım süresinden oluşan modelde fakülte öğrencisi olmak (Beta:0,158, %95GA: 2,92-10,14, p<0,001) ve sağlıkla ilgili basılı yayın takip etmek (Beta:0,144, %95GA: 2,08-8,49, p=0,001) siberkondri puanını etkileyen bağımsız faktörler olarak saptanmıştır.

Sonuç ve Öneriler: Üniversite son sınıf öğrencilerinin siberkondri düzeyi toplum ortalamasına göre yüksektir. Siberkondri düzeyi fakülte öğrencilerinde ve sağlıkla ilgili basılı yayın takip edenlerde daha yüksektir. Sonuçlar siberkondri düzeyinin formel ve informel sağlık hizmet kullanımını arttırdığını düşündürmektedir. Öğrencilerin güvenilir sağlık bilgisine erişimi sağlık personeli aracılığıyla verilecek danışmanlık hizmetleriyle ve sanal ortamdaki bilgilerin güvenilirliğinin arttırılmasıyla sağlanabilir.

İnternette tekrarlayan tıbbi bilgi aramanın bir sonucu olarak sağlık anksiyetesinin şiddetlenmesi için literatürde “siberkondri” terimi kullanılmaktadır. Siberkondri sıklığı ve olası risk etmenleri ile ilgili sınırlı sayıda çalışma bulunmaktadır. Bu çalışma, üniversite çalışanlarında siberkondri düzeyi ve ilişkili etmenleri belirlemek amacıyla yapılmıştır. Bu kesitsel çalışma 335 akademik ve akademik olmayan üniversite personeli üzerinde gerçekleştirilmiştir. Veriler, 17’si sosyodemografik özellikler ve sağlık hizmeti kullanımı ile ilgili ve 33’ü Siberkondri Ciddiyet Ölçeği sorusu olmak üzere 50 sorudan oluşan bir anket aracılığıyla toplanmıştır. Siberkondri ile ilişkili faktörleri değerlendirmek için çok değişkenli doğrusal regresyon kullanılmıştır. Katılımcıların ortalama siberkondri puanı 71,1±17,6'dır. Siberkondri puanı 35 yaş altı çalışanlarda daha yüksekti (p=0,001). Televizyonda sağlıkla ilgili yayınları izleyenlerde, hekim önerisi olmadan tetkik yaptıranlarda ve hekim önerisi olmadan ilaç kullananlarda siberkondri puanının daha yüksek olduğu bulunmuştur (p<0,05). Pamukkale Üniversitesi çalışanlarında siberkondri düzeyi orta düzeydedir. Yaş, TV’de sağlıkla ilgili yayınları izlemek, hekim önerisi dışında tetkik yaptırmak ve hekim önerisi dışında ilaç kullanmak siberkondri ile ilişkili etmenlerdir.

Objective: This study tested the hypothesis that large-scale provision of individual medical examination will reduce persistent anxiety about health and subjective health complaints after involvement in an aviation disaster with alleged exposure to hazardous chemicals. Method: Three measurements were performed: during the medical examination, 6 weeks later during consultation with the physician and 12 weeks after the first examination. Rescue workers (n=1736) and residents (n=339) involved in the disaster participated. Standardized questionnaires on health complaints and concerns were administered. Results: Both groups reported increased health anxiety and somatic sensitivity after 12 weeks. Residents reported more posttraumatic stress symptoms, whereas rescue workers seemed to have gained a better quality of life and were somewhat reassured. Participants who attended the consultation with the physician showed increased reassurance scores after 6 weeks, but their worries had increased again on follow-up. However, nonattendees reported more health anxiety on follow-up. More participants judged participation to have had a positive impact, instead of a negative impact, on their health. Conclusion: Our study does not indicate that a large-scale medical examination offered after involvement in a disaster has long-lasting reassuring effects and suggests that such examination may have counterproductive effects by sensitizing participants to health complaints.

The term "cyberchondria" has been used in the literature for the exacerbation of health anxiety as a result of searching for medical information on the Internet. There are limited studies on the prevalence and potential risk factors of cyberchondria. This study was carried out to determine the level of cyberchondria and related factors in university employees. This cross-sectional study was conducted on 335 academic and non-academic university staff. The data were collected through a questionnaire consisting of 50 questions, 17 of which were related to sociodemographic characteristics and healthcare utilization, and 33 of which were Cyberchondria Severity Scale questions. Multivariate linear regression was used to evaluate factors associated with cyberchondria. The mean cyberchondria score of the participants was 71.1±17.6. The cyberchondria score was higher among staff aged ≤35 (p=0.001). It was found that the cyberchondria score was higher in those who watched health-related broadcasts on TV, who received laboratory tests and procedures without a physician's referral and who took medication without a doctor's advice (p<0.05). The level of cyberchondria among Pamukkale University employees was moderate. Age, watching health-related broadcasts on TV, receiving laboratory tests and procedures without a physician's referral and taking medication without a doctor's advice were factors associated with cyberchondria.

An online survey (N = 210) is presented on how the perceived utility of correct and exaggerated countermeasures against Covid-19 is affected by different pronominalization strategies (impersonal form, you, we). In evaluating the pronominalization effect, we have statistically controlled for the roles of several personal characteristics: Moral Disengagement, Moral Foundations, Health Anxiety, and Embracing of Fake News. Results indicate that, net of personal proclivities, the you form decreases the perceived utility of exaggerated countermeasures, possibly due to simulation processes. As a second point, through a Structural Equation Model, we show that binding moral values (Authority, Ingroup, and Purity) positively predict both fake news embracing and perceived utility of exaggerated countermeasures, while individualizing moral values (Harm and Fairness) negatively predict fake news embracing and positively predict the perceived utility of correct countermeasures. Lastly, fake news embracing showed a doubly bad effect: not only does it lead people to judge exaggerated countermeasures as more useful; but, more dangerously, it brings them to consider correct countermeasures as less useful in the struggle against the pandemic.

The slippery patient’s position into the physician’s office. Groundwork for a pragmatic approach of health inquietude exposition

In this article, I offer to shed some new light on the sociological analysis of the patient-doctor encounter. To that end, I orient my approach on the examination of health-related inquietude exposition. I draw on ethnographic data, observations of patient-doctor encounters and interviews with patients. From the office threshold through the exit, the patient seems weaving. Even though, patient’s conditions are little problematic from the diagnostic point of view, the person, and her/his subject position, seem constantly tangent, tacking between different states. The scale of the patient’s difficulty is somehow surprising. S/he has to navigate both for communicating the medical trouble which simultaneously exposes him/her to several risks (e.g. not being taken seriously, not being examined with attention, suffering might not be alleviated, solicitude request might not be met) and for elucidating what is going on with this troubling biological body. I show how an analysis can be carried out on the subject’s arrangements with one’s inquietude and its exposition to the doctor. While focusing on the patient dynamics of exposition/being exposed, I provide insight for a little discussed space of reflection and inquiry.

Sur la base de l’observation d’échanges entre médecins (généralistes et spécialistes) et patient.e.s et du retour rétrospectif (lors d’entretiens) de ces dernier.e.s sur leur conduite, nous examinons des consultations médicales à partir d’une lecture pragmatique de l’exposition de l’inquiétude. Du seuil à la sortie du cabinet, les patient.e.s semblent louvoyer. Alors même que les situations rencontrées sont relativement peu problématiques du point de vue du diagnostic, la personne – et sa position de sujet – semblent constamment glisser entre différents états. L’ampleur de la difficulté du patient a quelque chose de surprenant. Il doit louvoyer tant pour communiquer un trouble qui l’expose simultanément à différents risques – ne pas être pris au sérieux, ne pas être examiné avec attention, sa souffrance ne soit pas soulagée, sa demande de sollicitude ne soit pas entendue – que pour élucider ce qu’il se passe avec ce corps biologique. En examinant les dynamiques par lesquelles il s’expose en inquiétude et s’en trouve exposé, nous ouvrons un espace de questionnements. Ce site d’enquête classique qu’est la rencontre médecin-patient permet de poser quelques jalons d’une réflexion socio-anthropologique plus large qui interroge les reconfigurations des conditions d'exposition et de traitement de l’inquiétude qui accompagnent l’essor de nouvelles médiations donnant accès à des informations relatives au corps en général (extensions d'Internet, cartes génomiques individuelles, applications de self-tracking).

Purpose: This study investigates individuals' health perceptions and health anxiety
within the framework of functional health patterns (FHP) during COVID‐19
pandemic.
Desing and Method: This study is a descriptive and cross‐sectional design. Data
were collected from participants (n = 3464) through a three‐part web‐based form.
Findings: Individuals' health anxiety and perceptions are at a medium level. The
participants’ health perception and health anxiety scale mean scores demonstrated
significant differences according to the FHP except for sexuality.
Conclusions: It can be stated that individuals use the anxiety they experience to be
vigilance against a potential health threat.
Practice Implications: Researchers can use findings in the examination of psychological
health problems that can be seen in individuals after the pandemic.

The exacerbation of health anxiety as the result of repeated online searches for medical information has been termed cyberchondria. Cyberchondria may lead to increased use of health services and increased healthcare costs. The aim of this study was to assess the validity and reliability of Turkish form of the Cyberchondria Severity Scale (CSS). This study was conducted on 335 employees who work in Pamukkale University. Depression Anxiety Stress Scale (DASS) was used for concurrent validity. After language adaptation and content validity of the CSS, confirmatory factor analyses (CFA) was performed. The mean age of participants was 38.2 ± 8.5 years and 64.5% of them were male. Cronbach alpha coefficient of CSS calculated for the reliability was 0.89, while the subscales ranged from 0.65 to 0.85. The correlation coefficients for test-retest reliability were ranged from 0.53–0.71. Correlation of CSS with DASS was r = 0.33. Fit indices based on CFA results were all excellent or within acceptable ranges. The Turkish version of CSS has adequate psychometric properties of validity and reliability, and can be used to assess cyberchondria.

Many studies have shown that cardiac anxiety when occurring in the absence of coronary artery disease is common and quite costly. The Cardiac Anxiety Questionnaire (CAQ) is an 18-item self-report measure that assesses anxiety related to cardiac symptoms. To better understand the construct of cardiac anxiety, a factor analysis was conducted on CAQ data from 658 individuals who were self or physician-referred for electron beam tomographic screening to determine whether clinically significant coronary atherosclerosis was present. A four-factor solution was judged to provide the best fit with the results reflecting the following factor composition: heart-focused attention, avoidance of activities that bring on symptoms, worry or fear regarding symptoms, and reassurance-seeking. Factorial invariance across groups was also assessed to determine whether the factor structure of the CAQ was similar in individuals with and without clear evidence of coronary atherosclerosis. The factor structure of the CAQ did not differ between the two groups. However, the group without coronary atherosclerosis had significantly higher mean scores on their attention and worry/fear factors suggesting that people without a diagnosed cardiac condition pay more attention to and worry more about their cardiac-related symptoms than those people who have coronary atherosclerosis. Objective: The coronavirus epidemic have had different effects on psychological aspects of individuals, especially on patients with corona disease. The aim of this study was to investigate the relationship between intolerance of uncertainty and health anxiety with cognitive emotion regulation strategies in outpatient with Corona disease. Method: For this purpose, a sample of 400 adult men and women outpatients over than 20 years old evaluated by health anxiety questionnaire (Salkovskis, 2002), intolerance of uncertainty (Freeston, 1994), and The Cognitive Emotion Regulation Questionnaire (Garnefski & Kraaij, 2001) via social network.  To analyze the data, Pearson correlation and path analysis were used. Results: The results showed health anxiety and intolerance of uncertainty were negatively correlated with adaptive, and positively with non-adaptive cognitive emotion regulation strategies.
