Dry eye disease

Opinion Dry eye disease (DED) is one of most common ocular disease and reason for primary eye care visit worldwide. DED has gone through major evolutions over the past couple of decades. As stated in the latest DEWS II report in 2017, the definition of DED is now "a multifactorial disease of the ocular surface characterized by a loss of homeostasis of the tear film, and accompanied by ocular symptoms, in which tear film instability and hyperosmolarity, ocular surface inflammation and damage, and neurosensory abnormalities play etiological roles" [1].

In March 2015, a meeting was held in London, United Kingdom, to address the progress in targeting the unmet need for dry eye disease (DED) treatment. The meeting, which launched the i2 = initiating innovation series, was sponsored by the Tear Film & Ocular Surface Society (TFOS; www.TearFilm.org) and supported by Dompé. The TFOS i2 meeting was designed to review advances in the understanding of DED since publication of the 2007 TFOS International Dry Eye WorkShop (DEWS) report, and to help launch the highly anticipated sequel, DEWS II. The meeting was structured to discuss the scope of the DED problem, to review the clinical challenges of DED, and to consider the treatment challenges of DED. This article provides a synopsis of the presentations of this TFOS i2 meeting.

Objective: To assess the safety and efficacy of the Pellevé Wrinkle Treatment System as compared to LipiFlow, for treatment of dry eye syndrome due to meibomian gland dysfunction (MGD). Materials and Methods: In this split face, randomized study, ten patients who met specific criteria for inclusion were followed after treatment with the Pellevé Wrinkle Reduction System (what the authors have dubbed the "thermaLid Procedure") and a standard LipiFlow procedure performed the same day, with 1 and 3 months follow-up. Pre- and post- treatment MGD was assessed using a variety of subjective and objective measurements.   Results: The function of the meibomian glands improved significantly from baseline to 3 months for both thermaLid and Lipiflow treated eyes, with no significant difference between them, as measured by MGE grading, wax plug scoring, Standard Patient Evaluation of Eye Dryness (SPEED) and Ocular Surface Disease Index (OSDI) scoring. To study the systemic safety and patient tolerability of frequent dosing of cyclosporine A (CsA) 0.05% eyedrops in the treatment of ocular surface disease. This is a retrospective case series. Patients with significant ocular surface diseases who were treated using topical CsA higher than the usual twice daily dosing (3-8 times daily and over a treatment period of 1-70 months). The main outcome measures are plasma levels of CsA and local tolerability. Symptom assessment, corneal staining using fluorescein, conjunctival staining using lissamine green, tear film breakup time, and other signs according to the disease process were monitored. Discontinuation of treatment due to intolerability was recorded. CsA levels were measured in the plasma at a clinical laboratory. Plasma levels of CsA were below the level of detection (7 ng/mL) in all the 41 patients included. This study investigated optometrists’ attitudes and self-reported practice behaviors towards omega-3 fatty acids for eye health, and knowledge and understanding of their potential risks and benefits. An anonymous online survey was distributed to optometrists in Australia and New Zealand. Questions included practitioner demographics and practice modality; self-reported practices and recommendations relating to diet, nutritional supplements, and omega-3 fatty acids for age-related macular degeneration (AMD) and dry eye disease (DED); and practitioner knowledge about omega-3 fatty acids. Of 206 included surveys, most respondents (79%) indicated recommending for their patients to consume omega-3 fatty acids to improve their eye health. Purpose: To compare the clinical aspects of patients with dry eye disease according to infection of Helicobacter pylori (H.pylori) using urea breath test which is non-invasive and find out the correlation of dry eye and H.pylori infection. Methods: We detected the infection of H.pylori using urea breath test (BreathTek®, Otsuka America Pharmaceutical, Inc., USA) in 52 patients with dry eye disease from August 2018 to December 2019 and analyzed their medical records. Slit lamp examination, Ocular Surface Diseast Index (OSDI) questionnaire were performed. Tear meniscus height was measured by optical coherence tomography (Spectralis, Heidelberg Engineering, Heidelberg, Germany). Lipid layer thickness, total blinking rate, partial blinking rate and meiboscore were measured by ocular surface interferometer (Tear Science, Morrisville, NC, USA). Results: The number of people who were positive for urea breath test was 17 (32.7%) and negative for test was 35 (67.3%). There were no differences in age, sex, tear meniscus height, lipid layer thickness, total blinking rate for one minute and partial blinking rate according to H.pylori infection. The OSDI scores (39.7, 28.5) and rate of severe loss of meibomian glands (23.5%, 4.3%) were significantly higher in the H.pylori positive group (p<0.05). The rate of patients diagnosed with Sjogren's syndrome (35.3%, 17.1%) was also significantly higher in the H.pylori positive group(p<0.05). presented mixed type of aqueous tear-deficient and evaporative type. Through detailed history taking for patients with dry eye, we could more actively suspect Sjogren's syndrome and conduct additional tests.
