Plasma glucose response to recreational diving in novice teenage divers with insulin-requiring diabetes mellitusA growing number of individuals with insulin-requiring diabetes mellitus (IRDM) dive, but data on plasma glucose (PG) response to diving are limited, particularly for adolescents. We report on seven 16-17 year old novice divers with IRDM participating in a tropical diving camp who had recent at least moderate PG control (HbA1c 7.3 +/- 1.1%) (mean +/- SD). PG was measured at 60, 30 and 10 min pre-dive and immediately following 42 dives. Maximum depth (17 +/- 6 msw) and total underwater times (44 +/- 14 min) were not extreme. Pre-dive PG exceeded 16.7 mmol x L(-1) (300 mg x dL(-1)) in 22% of dives. Males had significantly higher pre-dive levels (15.4 +/- 5.6 mmol x L(-1) [277 +/- 100 mg x dL(-1)] vs. 12.8 +/- 2.9 mmol x L(-1) [230 +/- 52 mg x dL(-1)], respectively) and greater pre-post-dive changes (-4.3 +/- 4.4 mmol x L(-1) [-78 +/- 79 mg x dL(-1)] vs. -0.5 +/- 4.3 mmol x L(-1) [-9 +/- 77 mg x dL(-1)], respectively). Pneumomediastinum after lung packingLung packing (glossopharyngeal insufflation) consists of forcing air into the lungs, using glossopharyngeal muscle contractions similar to swallowing. Breath-hold divers perform this technique after a maximal inhalation prior to diving, thus increasing initial lung volume. However, as suggested by previous authors, this breathing maneuver could theoretically lead to lung rupture. Here we report a pneumomediastinum found on chest CT scan in a diver during a physiological study, when glossopharyngeal insufflation increased the volume of gas in the lungs by 1,040 ml (over his total lung capacity); at the same time, his transpulmonary pressures increased up to 4.1 kPa. We discuss the possibility that the very high transpulmonary pressures during lung packing caused this pneumomediastinum.

Effect of varying deep stop times and shallow stop times on precordial bubbles after dives to 25 msw (82 fsw)In our previous research, a deep 5-min stop at 15 msw (50 fsw), in addition to the typical 3-5 min shallow stop, significantly reduced precordial Doppler detectable bubbles (PDDB) and "fast" tissue compartment gas tensions during decompression from a 25 msw (82 fsw) dive; the optimal ascent rate was 10 msw (30 fsw/min). Since publication of these results, several recreational diving agencies have recommended empirical stop times shorter than the 5 min stops that we used, stops of as little as 1 min (deep) and 2 min (shallow). In our present study, we clarified the optimal time for stops by measuring PDDB with several combinations of deep and shallow stop times following single and repetitive open-water dives to 25 msw (82 fsw) for 25 mins and 20 minutes respectively; ascent rate was 10 msw/min (33 fsw). This is the second report based on a survey of Divers Alert Network Asia-Pacific (DAN AP) members who dive with cardiovascular and respiratory conditions and diabetes. It examines the medical management of the divers' conditions, any diving modifications used to mitigate the risk and outcomes. An online cross-sectional survey was sent to 833 divers who had declared a targeted medical condition when applying for DAN AP membership between July 2009 and August 2013. Two-hundred-and-sixty-eight respondents (32%) provided sufficient information on their conditions to be included in the analyses. These included ischaemic heart disease (31), arrhythmias (20), cardiac septal defects (31), other cardiac conditions (10), hypertension (127), diabetes (25), asthma (40) and pneumothorax (5). With other multi-system diseases, such as septic shock, we optimise everything, we ventilate them, we maintain their cardiac output, we give them antibiotics, and intensive care nursing. But in spite of all our efforts there is a percentage of patients with septic shock who will die. I wonder whether it is the same with decompression sickness. Perhaps we are at a plateau now, with the best that we are going to get. Perhaps results in the future are not going to improve that much. Would anybody like to comment on that question?
