2009, Interactive cardiovascular and thoracic surgery
Right heart infective endocarditis presenting as an isolated apical mural mass is an extremely uncommon finding. A 24-year-old woman with congenital deafness and aphasia was admitted with recurrent attacks of fever and lobar pneumonia. Her past medical history was significant for an open operation for left nephrolithiasis five months before admission. She had no history of congenital heart defect, intravenous drug use or central venous line insertion. Diagnostic workup revealed a large pedunculated solid mass attached to the apex of the right ventricle and multiple septic foci in both lungs. Repeated blood cultures were negative. In spite of aggressive antibiotic therapy, she had progressively worsening respiratory distress. She was successfully operated for the mass and the pathologic findings were consistent with endocarditis. To our knowledge, the anatomical location of the mural endocarditis (apex of right ventricle) is a pretty uncommon condition.

Echocardiography is one of the most powerful diagnostic and monitoring tools available to the modern emergency/critical care practitioner. Currently, there is a lack of specific European Association of Cardiovascular Imaging/Acute Cardiovascular Care Association recommendations for the use of echocardiography in acute cardiovascular care. In this document, we describe the practical applications of echocardiography in patients with acute cardiac conditions, in particular with acute chest pain, acute heart failure, suspected cardiac tamponade, complications of myocardial infarction, acute valvular heart disease including endocarditis, acute disease of the ascending aorta and post-intervention complications. Specific issues regarding echocardiography in other acute cardiovascular care scenarios are also described.

This is the first report in the cardiac surgical literature in a grown-up congenital heart male patient with endocarditis of the Contegra conduit who developed septic shock with cardio-respiratory failure and required treatment with extra corporeal membrane oxygenation (ECMO) in order to stabilize his clinical condition preoperatively. 2009 Published by European Association for Cardio-Thoracic Surgery. Tricuspid annulus (TA) evaluation continues to be a major problem in the surgical decision-making process. Obviously, 2-dimensional transthoracic echocardiography (2D TTE) is limited in TA visualization due to its complex 3D shape. The study aimed to determine TA morphology, size and function with real-time three-dimensional echocardiography (RT3DE) in 40 patients divided into two equal groups (I: normal TA and II: dilated). 2D TTE measurements included TA diameter (TAD) at apical 4-chamber (AP4CH) and parasternal short axis (PSAX) views. RT3DE measurements included TA area (TAA), major TAD and minor TAD. TA fractional shortening (TAFS), and TA fractional area change (TAFAC) were calculated from end-systolic and end-diastolic measurements.
