RESUMEN Se evaluaron un total de 364 pacientes recién diagnosticados mediante la prueba de ELISA y confirmados por Western Blot para VIH. Dichos pacientes habían acudido a la UPRIC en el periodo agosto 2012 - agosto 2013. La presencia de infección activa por VHB se observó en 2,7% (10/364 muestras) de los pacientes infectados por VIH recién diagnosticados (estado basal), lo cual se evidenció por la detección del HBsAg y HBcAc en la muestra estudiada. Se obtuvo evidencia de infección pasada por el VHB en 16,8% (61/364) de las muestras analizadas definida por la presencia del HBcAc y ausencia del HBsAg. Con respecto a la coinfección VIH-HTLV I/II no se encontró y/o detectó ningún paciente. No hubo diferencias significativas entre los grupos considerados. Estos resultados se explican al observar no solo los valores promedio cercanos entre sí, sino además por la fuerte heterogeneidad o elevada varianza de las variables estudiadas. Debido al bajo porcentaje de pacientes coinfectados VIH/VHB obtenido, es necesario realizar estudios longitudinales que permitan evaluar ampliamente el efecto ejercido entre ambos virus. PALABRAS CLAVE: VIH, VHB, HTLV, coinfección ABSTRACT A total number of 364 patients newly diagnosed with the 4th generation HIV by ELISA and confirmatory Western Blot were evaluated. These patients came to UPRIC between August 2012 and August 2013. Active HBV infection was detected in 2.7% (10/364) of the newly diagnosed patients (basal state), through the HBsAg and HBcAc detection. Evidence of past HBV infection was observed in 16.8% (61/364) of the analyzed sample, defined by HBcAC presence and HBsAg absence. No patients with HIV- HTLV I/II co-infection were detected. There were no significant differences between the studied groups. These results can be explained by observing the close inter group average values and the high heterogeneity or variance of the studied variables. Due to the low percentage obtained of HIV/HBV co-infected patients, longitudinal studies are needed that allow wide evaluations of the effects exerted by both viruses. Se revisan y analizan los elementos teóricos inherentes a los elementos más importantes sobre la infección por el virus de la inmunodeficiencia humana en cuanto biología de los virus del VIH, elementos clínicos que intervienen en la evolución natural de esta enfermedad, uso correcto de los exámenes complementarios, respuesta al tratamiento, medicamentos antirretrovirales disponibles actualmente, así como criterios de comienzo de dicha terapia y su influencia para mejorar la calidad de vida de las personas afectadas.

[Clinical management of acute and chronic human immunodeficiency virus infection before starting antiretroviral treatment]The evaluation of new cases of HIV infection is relatively common in Spain, where several thousands of patients with new infections are diagnosed each year. Eighty per cent of them have a chronic HIV infection at the first clinical evaluation, which is symptomatic (late presenters) in up to 30% of patients. The initial evaluation of HIV infection is not only directed at determining the clinical, virological (plasma HIV RNA viral load, resistance test and viral tropism) and immunological (CD4+ T-cell cell count) situation of the patients, but must also address the study of their co-infections (hepatitis, tuberculosis) and comorbidities (cardiovascular, hepatic, renal and bone) and the risk of HIV transmission. This is needed in order to decide, whether or not to start antiretroviral treatment, and with which combined antiretroviral treatment to start with, the prophylaxis of opportunistic infections, and the treatment of coinfections and comorbidities.
