HPV-based strategy in follow-up of patients treated for high-grade cervical intra-epithelial neoplasia: 5-year results in a public health surveillance setting2016 •
Human papillomavirus (HPV) DNA testing is used increasingly for measuring the outcome of treatment for high-grade cervical intra-epithelial neoplasia (CIN2+). However, there is no international consensus regarding the number of tests and follow-up visits necessary in the post-treatment surveillance. A negative HPV DNA test result may permit relaxing the intensive post-treatment surveillance, but this possibility has not been standardized by all institutions to date. In 2008, the surveillance programme covering the Emilia-Romagna region in northern Italy adopted the HPV DNA test as a routine tool in the follow-up of women treated for CIN2+. Data from a prospective 5-year study are reported herein. Three hundred and ten patients treated for CIN2+ with a loop electrosurgical excision procedure underwent HPV DNA testing, cytology and colposcopy at 6 months post treatment. Cervical cancer control, priorities and new directions2004 •
Cervical cancer is caused by infection with a range of high risk “oncogenic” human papillomavirus (HPV) types, and it is now accepted that >99% of cervical cancer is initiated by HPV infection. The estimated lifetime risk of cervical cancer is nevertheless relatively low (less than 1 in 20 for most community based studies). Although sensitivity and specificity of the available diagnostic techniques are suboptimal, screening for persistent HPV infection is effective in reducing the incidence of cervical cancer. Infection can be detected by molecular techniques or by cytological examination of exfoliated cervical cells. Persistent infection is the single best predictor of risk of cervical cancer.1The latest findings of HPV and cervical cancer research need to be widely disseminated to the scientific and medical societies that are updating screening and management protocols, public health professionals, and to women and clinicians. This report reviews current evidence, clinical implications and directions for further research in the prevention, control and management of cervical cancer. Secondary Prevention of Cervical Cancer: ASCO Resource-Stratified Clinical Practice GuidelinePurpose To provide resource-stratified, evidence-based recommendations on the secondary prevention of cervical cancer globally. Methods ASCO convened a multidisciplinary, multinational panel of oncology, primary care, epidemiology, health economic, cancer control, public health, and patient advocacy experts to produce recommendations reflecting four resource-tiered settings. A review of existing guidelines, a formal consensus-based process, and a modified ADAPTE process to adapt existing guidelines were conducted. Other experts participated in formal consensus. Results Seven existing guidelines were identified and reviewed, and adapted recommendations form the evidence base. Four systematic reviews plus cost-effectiveness analyses provided indirect evidence to inform consensus, which resulted in ≥ 75% agreement. Политическая наука перед вызовами XXI века: деглобализация, деколонизация, демистификация / Political Science and the Challenges of the Twenty First Century: Deglobalization, Decolonization, Demystification2024 •
В статье предпринята попытка применить исторический подход для описания вызовов, с которыми сталкивается политическая наука в XXI в. Рассмотрение проблем политической науки ведется с позиций признания и понимания нелинейности процессов научного познания, с применением методологии «вызова-и-ответа», в рамках которой описываемые вызовы носят внешний для политической науки характер. В статье выделены три основные вызовы: деглобализация, деколонизация, демистификация. Каждый из них имеет собственный генезис, однако все они в значительной степени пересекаются и усиливают друг друга. Вызов деглобализации заставляет политическую науку пересматривать существенные научные достижения и результаты, основанные на явлениях и событиях последних 40 лет, в течение которых глобальные процессы казались всеобъемлющими и долговременными. Накопленный багаж проблем и провалов, связанный с либеральной моделью глобализации, делает эту задачу необходимой, но и весьма масштабной. Деколонизации знания ставит под сомнение гигантский объем знаний, выработанный в период становления и развития колониальной системы. Политическая наука пока не восприняла этот вызов во всей его сложности, но этот выбор придется сделать в ближайшее время. Демистификация, радикально отражающая противостояние научной истины и «удобного знания», обращает внимание на то, что «групповые эпистемологии» в современном обществе часто становятся важнее научного знания. В статье ставится вопрос о том, найдутся ли силы внутри политической науки, чтобы заметить обозначенные вызовы, трезво их оценить и профессионально на них ответить.
