Drug Compliance and Drug adherence describe the behaviour of patients following advice or
instructions on the use of pharmacological agents. In the Pharmacology, compliance refers to
drug intake, but medically may also include adherence to diet, lifestyle, and other therapeutic
modalities including medical follow-up. Noncompliance is the failure to fill the prescription,
failure to take the prescribed drug, and incorrect frequency, timing, or dosage of drug
administration. The consequences of poor adherence to long-term therapies are resistance,
poor health outcomes, low quality of life, increased health care costs, morbidity and death.
Poor adherence to treatment of chronic diseases is a worldwide problem of striking
magnitude and the impact of poor adherence grows as the burden of chronic disease grows
worldwide. Health systems must evolve to meet these new challenges. Patients who fail to
adhere to drug regimen must be supported, educated, encouraged and not blamed. Health
professionals need to be trained in adherence because adherence is an important modifier of
health system effectiveness. Thus, increasing the effectiveness of compliance and adherence
interventions may have a far greater impact on the health of the population than any
improvement in specific medical treatments or any pharmacological modifications.

Heart failure is the inability of the heart to pump blood to meet the body requirements or can do so only at high pressures. This may due to Systolic heart failure with reduced ejection  fraction or diastolic heart failure due to the heart is unable to relax or both when it is biventricular. The prevalence of heart failure (HF) has increased globally in recent decades.
Advances in our understanding of underlying pathophysiologic mechanisms have given rise  to new therapies for treating the growing heart failure population. Nonetheless, morbidity and mortality associated with heart failure and its financial implications are daunting. Thus, novel therapies that can improve the condition of heart failure patients are urgently needed. Three new agents Ivabradine, LCZ696 and Patiromer have been recently been approved by the FDA and other novel agents which have shown considerable promise in early studies, omecamtiv mecarbil, finerenone, and Serelaxin are currently being evaluated in late-phase clinical trials, while two other drugs, ularitide and TRV027, are also being evaluated in ADHF patients.
Patiromer and zirconium cyclosilicate are attractive agents that are expected to prevent hyperkalemia during renin-angiotensin-aldosterone system inhibition, and serelaxin and  urodilatin are promising drugs in the treatment of acute heart failure.
