Background: The objectives of this study were first to show principles of the minimally invasive video-assisted thyroidectomy (MIVAT), based on a video highlighting critical steps, and second to discuss tips and pearls to assist surgical teams that would like to start using this technique. Methods: Based on a video, we described tips and pearls of a MIVAT. Results: MIVAT includes 5 main steps: (1) skin incision and identification of the common carotid artery, (2) dissection and ligation of the upper pedicle, (3) identification of the inferior laryngeal nerve and parathyroid glands, (4) isthmectomy and lobe extraction, and (5) closure. Discussion: Coordination between the surgeon and the 2 assistants is of paramount importance for the performance of MIVAT. Appropriate material is also required. The magnification and tissue contrast emphasizes the identification of the vessels, the superior and inferior laryngeal nerves, and parathyroid glands, on a large-view screen.

Aim: We aimed in this randomized clinical trial to compare the results of traditional versus stapled hemorrhoidectomy for treatment of third and fourth degree hemorrhoids. Methods: Thirty patients admitted for surgical treatment of prolapsing hemorrhoids were randomly assigned to traditional (n=15) or stapled hemorrhoidectomy (n=15). All patients received standardized preoperative and postoperative analgesic and laxative regimens. Postoperative pain measured by Visual Analog Scale (VAS) was used as the primary outcome measure. Secondary outcome measures were; operative time, use of analgesia, postoperative complications, hospital stay duration, time to first bowel motion, and return to normal activity. Results: Stapled procedure for hemorrhoids is associated with a significant improvement in postoperative pain control and with an earlier return to normal activity. Operative time and duration of hospital stay were shorter for the stapled procedure. . A trend towards earlier bowel fun...

The term Bezoar is derived from the Arabic "gadzehr" or the Persian "padzahr" both meaning counterpoison or antidote. The removal of trichobezoars is mandatory because of the risk of potentially life-threatening complications such as intestinal obstruction, gastric bleeding, and perforation. Gastric Trichobezoars are unusual cause of outlet obstruction in the emergency department. . The surgical treatment is usually the first choice of treatment in a big trichobezoar [or big trichobezoars]. A 21-year-old woman presented to the outpatient clinic with upper abdominal pain, nausea and epigastric swelling, and with a past history of depression. Abdominal ultrasound showed echogenic mass within the stomach. Abdominal CT showed a mass in the stomach, endoscopically the mass was diagnosed to be a hairball.

Case Report: The author reports a young adult girl complaining of a painless swelling just above the vaginal introitus after being circumcised. The girl’s family history showed two sisters, 22 and 23 years old, with the same complaint treated by the author 12 months before by simple excision of the swellings. Female genital mutilation is not recommended by any religious or medical opinion and is a violation of human rights against helpless individuals. The potential physical complications resulting from the procedure are numerous, one of the most important being epidermal inclusion cysts within the cicatrix.

Background: The importance of early recognition of hemorrhagic shock and its effects on outcome have long been recognized. Traditional vital signs are relatively insensitive as early diagnostic markers of hemorrhage. The Shock Index (SI); Heart Rate (HR) divided by Systolic Blood Pressure (SBP), has been suggested as such a marker. We tested the accuracy of the SI in differentiating major from minor injury in trauma patients. Aim: To assess accuracy of shock index as a simple and early predictor marker in differentiating major from minor injuries in trauma patients in Emergency Department at Suez Canal University Hospital. Patients and Methods: This cross sectional study was conducted on 106 trauma patients, both genders and all age groups above 18 years old, admitted to Emergency Department at Suez Canal University Hospital. Demographics, injury mechanism, HR, SBP, base deficit and lactate were recorded and Injury Severity Score were calculated. Major injury was defined as either d...

Aim: To compare the effectiveness of local glyceryl trinitrate (GTN) versus internal sphincterotomy in the management chronic anal fissure. Methods: Eighty patients with chronic anal fissure were enrolled in the study. They were divided into two groups; group (1) included 40 patients treated with topical GTN 0.2% on liposomal base applied to the anoderm twice daily and group (2) included 40 patients treated with internal sphincterotomy. We compared the effectiveness of both techniques in the management chronic anal fissure Results: In group 1, healing of fissures occurred in 85% of patients after 8 weeks therapy. Headache as a side effect developed in 65% of patients. In group 2, healing occurred in 97.5% of patients after 8 weeks. Incontinence to flatus occurred in 3 patients (7.5%), mild soiling in 2 patients (5%) and one patient developed wound infection. All complications were temporary except for one patient with persistent incontinence to flatus. At the end of 8 weeks both gro...

Introduction: acute appendicitis is the major surgical abdominal disease in emergency departments and is also among the five leading causes of litigation against emergency physicians. Delayed diagnosis of appen-dicitis is more likely to occur in patients, who present atypically, and those lack a thorough physical exami-nation, or those received intramuscular narcotic analgesia. The aim of this study was to study the effect of delay in diagnosis of acute appendicitis as regard postoperative findings, length of hospital stay and post-operative complications. Patients & Methods: patients with complicated appendicitis were subjected to the present study and treated by a single surgical team. The parameters of our study were incidence of perfora-tion or gangrene at surgery, length of stay and post-operative complications. Results: the present study showed that delay in diagnosis of acute appendicitis is associated with a more advanced stage of disease and a higher morbidity. Conclusion: ...

We aimed to test the hypothesis that Minimally Invasive Video-assisted Thyroidectomy (MIVAT) affords comparable safety and efficacy as to the open conventional surgery, when dealing with patients with unilateral thyroid nodules or follicular lesions, in terms of cosmetic results, intraoperative and postoperative complications, postoperative pain and hospital stay. This was a single-blinded randomised controlled trial comparing the MIVAT with conventional thyroidectomy. The primary endpoints of the study were measurement of postoperative pain after 24 and 48 hours from operation and self-rated patient satisfaction with cosmetic outcome three months postoperatively. The secondary outcome measures were operative time, incidence of temporary and permanent recurrent laryngeal nerve injury, postoperative haematoma formation, length of incision, and duration of hospital stay. Operative time was significantly less with open thyroidectomy than with MIVAT, while MIVAT was associated with less...

pain control and with an earlier return to normal activity. Operative time and duration of hospital stay were
shorter for the stapled procedure. . A trend towards earlier bowel function after the stapled procedure, although not
significant in this study, would be consistent with less perianal pain and spasm.
Conclusion: Stapled hemorrhoidectomy is an effective treatment for third and fourth degree hemorrhoids
with significant advantages for patients compared with traditional hemorrhoidectomy.
